Job overview
We are looking for an experienced band 6 mental health practitioner to join our exciting HTT. Post holders will work within one of the home treatment team based at Farnham road hospital. As a Band 6 you will be working alongside both junior and senior practitioners. You will take a lead in coordinating shifts, delegating to other team members as well as liaising with other key stakeholders. You will be required to urgently respond to request for assessment and treatment of people experiencing a mental health crisis. You will work autonomously as well as an effective team member.
The post holder will be responsible for creating an environment which is safe, supportive and an easily accessible with a no wrong door approach.
Main duties of the job

Adhere to the code of practice/conduct of your registering body at all times
Participate in the multi-disciplinary assessment of individuals with mental health problems or vulnerabilities living in the community, by providing a community mental health perspective and expertise.
Participate in the provision of community mental health advice and consultation to colleagues, community groups and organisations.
Be responsible for the full assessment of the psychiatric/psychosocial needs of those patients who have been referred to the service; including the comprehensive assessment of risks to self and others
Work as a member of the MDT in assessing and planning care and responding to changes in people who use our services.
Ensure provision of therapeutic activities, emotional support and stability to clients to ensure effective engagement whilst maintaining professional boundaries.

Detailed job description and main responsibilities
· Ensure detailed and appropriate records are maintained to demonstrate that contact with people who use services, and their carers’, is meaningful and effective.
· Utilise a range of specialist skills appropriate to the needs of individual patients and the clinical setting in which they are required to be seen and work autonomously.
· Co-ordinate and monitor the delivery of person centred healthcare to a designated group of people who use our services, using complex clinical decision making where appropriate.

Essential criteria
- A professional mental health qualification e.g. RMN,OT, Social Work Degree or equivalent Clinical Supervision qualification
Experience

- 2 years post qualification experience

- Access to a vehicle which can be used for work purposes or ability to travel as required to meet the needs of the service

You must have appropriate UK professional registration.
