Job overview

We have an exciting opportunity for a pharmacist with drive and enthusiasm to join our Frailty Pharmacy team as Lead Specialist Pharmacist – Frailty and Adult Community Services.

We are looking for an experienced pharmacist with extensive clinical pharmacy knowledge and excellent communication skills. You will need to have the ability to work within a multidisciplinary team, driving change in practice and have a keen interest to actively participate in the training and development of others.

The department is supportive of further education, including leadership and prescribing qualifications if not already held, and you should be keen to continue your professional development.

Successful applicants will be required to participate in late night, weekend working and public holiday work. 

Main duties of the job

The post holder will work closely with the Principal Pharmacist for Frailty and Integrated Care and be embedded in the multidisciplinary team whilst being responsible for providing comprehensive clinical pharmacy services as well as being at the forefront of service development.

Detailed job description and main responsibilities

  • To work in partnership with the Chief Pharmacist and all other pharmacists to ensure safe, effective, economical and timely pharmaceutical care for all patients receiving a pharmaceutical service from the department.
  • To lead, provide, develop and evaluate clinical pharmacy services to meet the needs of the relevant speciality business units (SBU) in accordance with objectives set by the Principal Pharmacist – Frailty and Integrated Care in order to meet Trust objectives and the Clinical Governance Agenda.
  • To lead by example within the speciality to ensure a consistently high quality clinical pharmacy service is delivered to all clinical areas of the relevant SBU in collaboration with the multidisciplinary team.
  • To plan and implement under the direction of the Principal Pharmacist-Frailty and Integrated Care the strategic direction of the clinical pharmacy service provided to the Trust and lead on delivery within the relevant SBU(s).
  • To be a resource for the whole department and the Trust in your specialist clinical pharmacy area.
  • Dispense and supply medicines and provide medicines advice.
  • Provide relevant teaching/training/educational sessions under the direction (not supervision) of the Principal Pharmacist – Frailty and Integrated Care.
  • Deputise for the Principal Pharmacist – Frailty and Integrated Care where the service requires.
  • Provide a cover for the clinical pharmacy provision to other wards when the service requires.
  • Responsible for leading, developing and evaluating the delivery of clinical pharmacy services for designated Speciality Business units.
  • Manage the clinical pharmacy service provided to the named SBUs provided by a team of pharmacists and Medicines Management Technicians (MMTs)
  • Post holder is required to provide cover for the clinical pharmacy provision (and supervision) to other wards when the service requires (Relevant to experience).
  • To provide specialist in-house teaching, relating to the specialty, to Trust staff and to rotational pharmacists undertaking the Diploma in pharmacy practice.
  • Responsible for reviewing and writing procedures and policies relating to the relevant SBU/DCSS having an impact on staff within pharmacy and the wider trust.
  • Post holder is required to tutor a pharmacist undertaking the Diploma in pharmacy practice.
  • Post holder may be required to tutor a pre-registration pharmacist as the need arises.

Person specification

Qualifications

Essential criteria
  • Masters degree in pharmacy or equivalent and 1 year pre-registration training
  • Registered Pharmacist with the General Pharmaceutical Council (Great Britain) as attained following successful completion of 1 year competency assessment and examination.
  • Post-graduate Diploma in Clinical Pharmacy or equivalent- or near completion
Desirable criteria
  • Registered supplementary/independent non-medical prescriber- or aim to start the course within first year of the role
  • MSc in clinical pharmacy

Knowledge & Experience

Essential criteria
  • Professional knowledge acquired through vocational Masters degree in pharmacy (4 years) or equivalent
  • Advanced level core therapeutic knowledge and clinical skills as gained through Post-graduate clinical diploma (or equivalent)
  • Demonstrates high level of clinical knowledge, clinical reasoning and judgement; manages difficult and ambiguous problems
  • Understanding of background and aims of national and local healthcare policy/guidance and appreciate the implications of this on engagement and priorities
  • Specialist clinical knowledge and skills relevant to the area of practice as developed through experience and CPD including conferences, courses etc.
  • Demonstrable experience as a specialist practitioner in hospital clinical pharmacy
  • Experience as an advanced level clinical pharmacist practitioner, able to demonstrate a high level of competency e.g. equivalent to achieving Excellence in the majority of competencies in all clusters within the Advanced Level Competency Framework, except Management and R&D, which should be at Foundation level (as a minimum).
  • Previous specialist clinical pharmacy experience within medical specialties
  • Variety of clinical practice including medicine and surgery as a specialist
  • Evidence of working effectively within multidisciplinary groups
  • Previous experience of delivering training and education at post-graduate level
  • Evidence of activity in clinical audit or drug use review to improve practice
  • Knowledge of current developments in pharmacy
  • Knowledge of changes occurring in the NHS
Desirable criteria
  • Experience in Hospital pharmacy at management/directorate level
  • Experience of supervision or line managing others
