This is a new post required to expand Hepatology services for the effective delivery of a robust HCC surveillance pathway. The Primary purpose of the role is to deliver the HCC surveillance pathway including patient assessment, investigations, review of blood, USS, and lifestyle advice and signposting, The post holder will work closely with the HCC surveillance data manager and pathway navigator to co-ordinate care for patients engaged in hepatocellular cancer surveillance pathways.
Working with existing team members, the post holder will deliver nurse lead clinics for people with stable cirrhosis and channel patients with more advanced liver disease to Consultant clinics. The nurse will be accountable for requesting and co-ordinating six monthly diagnostics for patients with cirrhosis, and will work closely with the pathway navigator to track patients through the pathway.
The Trust will be required to develop a database of patients on surveillance (in future there will be local and national patient level reporting). In the set up phase the CNS will work with the Hepatology consultants to ensure that the RSCH complies with Key Performance Indicators defined by the Cancer Alliance.
The CNS will work closely with all the Consultant Hepatologists Liver CNS team, pathway navigators, cancer services and diagnostic services to retain patients in cancer surveillance pathways.
Main duties of the job
KEY RESULT AREAS/MAIN DUTIES AND RESPONSIBILITIES:
• To provide nurse led HCC surveillance clinics for patients with stable cirrhosis.
• Provide advice, information and support for participants enrolled in the liver HCC cancer surveillance pathways.
• Be responsible for care delivery for patients undergoing liver surveillance for cirrhosis and to meet the needs of patients.
• Be competent to:
v Undertake phlebotomy
v Autonomously support patients to make informed decisions prior to undergoing a surveillance USS.
v Communicate test results and implement surveillance and follow-up
• Manage patients within the liver cancer surveillance pathway in accordance with relevant society and National guidance (e.g., NICE QS152).
• To use specialist knowledge and advanced communication skills with patients and carers when obtaining consent for procedures and providing psychological support where necessary
• Acting as the patient advocate supporting them to navigate and make
informed choices throughout the process.

Detailed job description and main responsibilities

Hepatocellular Carcinoma (HCC) surveillance clinical nurse specialist
Band 6 – Fixed Term
37.5 hrs
We are looking to appoint an HCC clinical nurse specialist (CNS) to join the gastro/hepatology team at The Royal Surrey County Hospital. This is a great opportunity to be part of a friendly, dynamic, and forward-thinking team specialising in the treatment and holistic care of liver patients from across Surrey.
The role is to support the HCC surveillance programme, ensuring patients at high risk of developing liver cancer are assessed in a timely way and have full understanding of their surveillance schedule. The post-holder will implement a robust surveillance programme, education, and engagement of patients as well as track surveillance outcomes/establish prompt treatment. This will be a nurse-led service and will enable delivery of HCC surveillance for at risk patients on an ongoing basis through an education and engagement workstream. The CNS will act as a single point of contact for patients (and carers) throughout every stage of their pathway, supporting the delivery of a seamless, high quality and efficient service for patients. The CNS will work closely with the hepatology consultants, the HCC CNS, and the HCC network coordinator.
Ideally, the CNS will have had experience with working with liver patients and have knowledge and understanding of an HCC surveillance programme. You will need to have excellent communication skills-both written and verbal and excellent organisational skills.

Essential criteria
- Knowledge of specialist areas relevant to this post acquired through experience in clinical settings e.g., Hepatology inpatients or outpatients, drug or alcohol services, cancer care or research. State the specific knowledge
- Experience of independent practice assessing patients in a clinical area.
- CPD - Evidence of post qualifying and continuing professional development
- Must understand the background to and aims of current healthcare policy/national guidance/CQC/ and appreciate the implications of this on engagement for this project
- Understanding of the importance of informed consent
- Clinical Governance and risk management
- Working in a multi-disciplinary team environment
- Ability to manage own caseload in accordance with Screening targets, NHS cancer targets and working across multiple sites.
Desirable criteria
- Knowledge of Liver disease management including relevant diagnostic investigations
- Understanding of Cancer Pathways relevant to liver disease surveillance and management
- Research and or audit experience
Qualifications
Essential criteria
- Experienced healthcare professional e.g. Registered nurse with current NMC PIN Qualifications or current research practitioner with extended scope of practice including patient facing activities.
Desirable criteria
- Mentor/Teaching/Assessing qualification
- Qualification in advanced communication skills
