Come and join our award-winning team! In June 2023, the FHFT Virtual Frailty Ward (VFW) / Urgent Community Response (UCR) team was nominated for a Parliamentary Healthcare Award and a FHFT Facing the Future Award.

We specialise in managing acute frailty crises, sepsis, and exacerbations of long-term conditions, aiming to reduce unnecessary hospital admissions and promote patient choice in care delivery. Based at Frimley Park Hospital, our team operates across Surrey Heath, Northeast Hampshire, and Farnham, working within the Frimley Health and Care Integrated Board (ICB) framework.

Our team provides an innovative, responsive, and patient-centered service that delivers urgent community response and acute hospital-level treatment in patients' own homes. Our unique hybrid model ensures a rapid, two-hour response time, with referrals received directly from paramedics, GPs, NHS 111, community healthcare professionals, and hospitals. Patients are assessed, diagnosed, and treated in the comfort of their own homes.

 

Main duties of the job

  • The UCR MTA will play a key role within the UCR team, working closely with both the medical and therapy teams to provide essential support.
  • They will respond to emergency call-outs in the community, attending patients alongside either the medical and/or therapy team, depending on the patient’s needs, to ensure timely assessment and intervention.
  • The MTA will perform a range of extended clinical skills, including but not limited to vital signs monitoring, cannulation, venepuncture, 12-lead ECGs, and urinary catheterization.
  • They will also actively participate in the emergency response team, contributing to the delivery of high-quality urgent care

Detailed job description and main responsibilities

As an MTA, you will work closely with both the medical and therapy teams, providing essential support during emergency call-outs in the community.

Your responsibilities will include:

  • Attending patient visits alongside medical and/or therapy professionals.
  • Conducting timely patient assessments and reviews.
  • Performing extended clinical skills, including vital signs monitoring, cannulation, venepuncture, 12-lead ECGs, and urinary catheterisation.
  • Participating in the emergency response team.
