Working for our organisation

The SWLEOC is an NHS Treatment Centre providing regional elective orthopaedic surgery services (including inpatient, day-case and outpatient). Established by the four South West London acute Trusts to deliver strategic change in the delivery of planned orthopaedic care, the SWLEOC provides high quality, cost efficient, elective orthopaedic services ranked amongst the best in the world.

 

Since opening in January 2004, the SWLEOC has earned a reputation as a centre of excellence for elective orthopaedic surgery with excellent outcomes, low complications and high patient satisfaction. It has consistently achieved operational targets such as the 18 week patient wait time. Length of stay, infection rates and Patient Recorded Outcome Measures (PROMs) are amongst the best in the world. As a centre of excellence, the SWLEOC has a large volume of visitors from around the UK and abroad, looking at processes and outcomes.

Detailed job description and main responsibilities

1.     To provide a comprehensive pharmaceutical service, working towards expert level knowledge.

2.     To further develop managerial skills and expertise by working to objectives agreed with the Principal Pharmacist SWLEOC and the Deputy Chief Pharmacist.

3.     To liaise closely with the Principal Pharmacist SWLEOC to contribute to the maintenance and development of evidence based treatment guidelines/shared care protocols/patient group directions (PGDs), where appropriate, for the division and contribute to Pharmacy / Trust policies as required.

4.     To work in liaison with the pharmacy leads to ensure changes to practice meet required standards and are applied Trust-wide.

5.     To deputise for the Principal Pharmacist SWLEOC as necessary.

6.     To monitor the pharmacy service to the SWLEOC, working with the pharmacy leads to initiate improvements or changes as required.

7.     Appropriately escalate issues that may impact the delivery of routine pharmacy services delivered to SWLEOC.

8.     To report and investigate errors, complaints and incidents in line with Trust and departmental procedure.

9.     To facilitate the sharing of learning from errors, complaints and incidents

10. To undertake risk assessments and manage and report risk as required.

11. To attend/participate/present in departmental Clinical Governance meetings as appropriate.

12. To produce rotas for dispensary, ward cover and locking up.

13. To undertake dispensing/checking of prescriptions for patients using the Pharmacy and Trust Computer Systems and in accordance with departmental policies and procedures

14. To participate in the ward/clinical pharmacy service to wards to monitor prescriptions.

15. To participate in the daily Consultant led Multi-Disciplinary Team (MDT) ward rounds on the SWLEOC ward departments, providing expert pharmaceutical advice.

16. To undertake medicines reconciliation for each patient on admission to ensure prescribed medications are appropriate as early as possible in their stay.

17. To pharmaceutically screen medication drug charts and prescriptions to a safe and legal standard thereby ensuring the appropriate, cost-effective, and timely supply of medicines.

18. In a timely manner, pharmaceutically review and validate the medicines reconciliations completed by Clinical Pharmacy Technicians and act on all relevant pharmacy handovers.

19. To work with the clinical pharmacy technicians and assistants to ensure the provision of a seamless and efficient patient centred service

Person specification

QUALIFICATIONS / EDUCATION

Essential criteria
  • Registered Pharmacist with General Pharmaceutical Council
  • Pharmacy degree – MPharm Pre-registration training
  • Diploma in Clinical Pharmacy or equivalent
Desirable criteria
  • MSc in Clinical Pharmacy Practice or equivalent. Management training

EXPERIENCE

Essential criteria
  • Post registration UK hospital experience Demonstrate evidence of excellent clinical knowledge and skills Experience of providing services to a broad variety of patients Medicines Information
