Job overview

A very exciting role has become available in our Senior Leadership team within the Community and Older People’s Medicine Directorate here at Frimley Health. We have an opportunity for a dynamic Senior Clinician to join us as our new Consultant Practitioner for our Frailty Teams within the Older People’s Medicine Directorate.  The role will involve working across the frailty pathway, virtual ward, acute frailty and an acute community focus.

You will report to the Head of Nursing in the delivery of high quality patient care, promote patient safety and good patient experiences on the acute wards. You will work alongside the Deputy Head of Nursing to support the delivery of our strategic quality ambitions within the Directorate.

The post is an ideal opportunity for an experienced Senior Clinician to build upon and advance their leadership, clinical and managerial skills whilst being supported by the Divisional Head of Nursing, Nurse Consultant for Frailty, other Clinical Matrons, Ward Sisters, Clinical Senior Nurses and Service Manager to deliver quality care.

Main duties of the job

Ensure practice development for new recruits and existing staff.

 

Ensure that the service delivers against national targets and CQUINs.

 

Support the Older People’s wards at Frimley, the post holder will work with the Ward Managers for teaching and training on frailty and patient experience.

 

Be responsible for development and application of systems, control processes and risk management arrangements. 

 

To ensure full compliance with internal and external governance procedures such as audits, complaints and staff development against best practice requirements.

 

You will lead the drive to transform our services, delivering continuous improvement projects across the Older People’s Medicine, leading process review, reviewing operational performance, and seeking effectiveness and efficiency through robust reviews audits and action plans.
