We have an exciting opportunity in West Suffolk Hospital as a Social Prescriber within the Respiratory Physiotherapy team.

Working at West Suffolk Foundation Trust as part of the Alliance, you will work with people, clinicians, charities and across the public sector to develop and deliver the Alliance vision.

As set out in the NHS Long Term Plan, personalised care allows individuals to have a choice and control over how they receive healthcare, focusing on the individuals needs and wants.

 We are looking for a motivated team player who will support the design, development and delivery of the Respiratory social prescribing service as part of the well established social prescribing team  in West Suffolk Hospital working in partnership with community based social prescribing services. You will have the ability to work in a fast-paced environment, whilst understanding and supporting the needs of respiratory patients in West Suffolk.

The post holder will have a passion for personalised care and working with people. What matters to the individual rather than what is the matter with them should be at the heart of the role. Helping patients to improve their health, wellbeing and social welfare by connecting them to community services.  The post holder will facilitate One Off Personal Health Budgets to support discharge and admission prevention. 

 

You will have a strong professional track record of working in partnership to make a real difference to local communities. The successful candidate will have excellent relationship building, communicating and influencing skills, as well as an in-depth understanding of community development approaches. Experience or an awareness of working in a health and care environment would be beneficial.

 

This role can make a real difference to the community and offers an exciting challenge.

 

This role will be considered as a fixed term contract or secondment (in agreement with your line manager).

STAFF CAR PARKING

Staff Car Parking on the West Suffolk Hospital Site is eligible for individuals working outside of the core hours, 07:00 – 18:30. 

For individuals working within the core hours, the Trust offers parking off site at the Bury St Edmunds Rugby Club, which is serviced by a regular shuttle bus to and from the Hospital.  Details of the bus timetable can be found in the on-line starter pack on the West Suffolk Hospital website.

 This parking restriction does not apply if an individual is required to work on a weekend or if they are required to complete “on call” duties within their role.

  • Significant experience in health coaching.
  • Awareness of issues faced by people in their communities and local resources.
  • Excellent IT skills and ability to input data
  • Knowledge of Social Prescribing.
  • Experience of facilitating and supporting participants with complex and/or multiple barriers to success in health and social care settings.
  • Relevant training in adult safeguarding practices.

  • Frequent Travel Essential - You will need to travel, so you must either hold a full, current driving licence and have access to personal transport or meet the mobility requirements of the role through other reasonable and suitable means.
  • Good IT skills, using Microsoft Word, Excel, Powerpoint, Outlook, databases, and the internet.
