Band 4 Rehab Assistant Practitioner – Stroke

Integrated Therapies  Permanent – 35 hours per week

 An exciting opportunity has arisen for a Band 4 Rehabilitation Assistant Practitioner to support both Occupational and Physiotherapy on our acute Stroke ward at the West Suffolk Hospital.

The stroke service at West Suffolk Hospital consistently performs highly against the Sentinel Stroke National Audit Programme (SSNAP) which is a marker of standards of care.  The band 4 role is crucial to the success of the therapy team in achieving these standards. 

 As this is an acute role it will be ward based and will include work with a variety of patients and conditions day to day.

We are a friendly team and work closely with the wider multidisciplinary team to get the best outcomes for our patients.  The successful candidate will also work with other rehabilitation assistants and practitioners as part of the role.

 We provide in-house training and development opportunities for all staff and view this as a priority.

Prif ddyletswyddau'r swydd

·       You will complete a range of assessments including motor, cognition, vision, mood, function, co-ordination, balance, mobility with less complex stroke patients.  This will be under the supervision of a Physiotherapist and/or Occupational Therapist.

·       You will formulate treatment programmes, goals and discharge plans for your designated caseload under the supervision of a Physiotherapist and/or Occupational Therapist.

·       You will work jointly with the physiotherapists to support the rehabilitation of more complex patients.

·       You will complete a wide range of daily living and activity based tasks to support the occupational therapists.

·       You should be able to take direction from a variety of healthcare professionals and work as part of an integrated team.

·       Organisational skills, time management and flexibility are crucial in this environment.

·       We provide a 7 day service so the ability to work weekends will be required. 

·       Advanced communication skills will be necessary due to the patient group and fast pace of the environment.

·       A new direction for the stroke rehabilitation team is to run daily activity groups and it will be the role of the band 4 to support and deliver these groups.

JOB PURPOSE:

Under the supervision of a registered practitioner, the post holder will be expected to: 

  • Manage a defined non-complex caseload using evidence based patient centred principles to assess, plan, implement and evaluate interventions.
  • Use a person-centred approach in the planning, implementation, evaluation and monitoring of agreed interventions following guidelines and protocols promoting the functional fitness for discharge of patients to a level that is safe and consistent with their abilities, preferences and cultural beliefs.
  • Under the guidance of qualified professional staff, plan and implement interventions for complex patients to facilitate functional fitness.
  • Promote holistic and inclusive rehabilitation, incorporating occupational therapy, physiotherapy and speech and language principles to support patients in their recovery programmes.

  • • GCSE Maths and English Grade C or above
  • • Can demonstrate intermediate level of theoretical knowledge and clinical reasoning skills acquired through coursework, in-house training and experience
