Job overview

An exciting opportunity for a band 5 Community Nurse has arisen in the Bury Rural Integrated Health Team. The Team works collaboratively within a wide range of other health professionals and agencies including Adult Social Care and GP practices in a fully integrated way. The successful applicant will join a dynamic, friendly team of community nurses and therapists. You will be providing nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multi-disciplinary team including the Community Matrons and Interface Geriatrician.

Main duties of the job

You will be using a wide range of nursing skills and will need to be confident in your assessment skills and in your ability to provide safe, effective treatments whilst working in a community setting. Experience in palliative care, wound management and catheter management will be an advantage. You will have the chance to be fully involved in working with the MDT and integrating with ACS, hospice and hospital staff and will need to be committed to ongoing learning and development in the team. This will be the perfect opportunity if you would like a role where you can use a broad range of nursing skills.

We would expect the applicant to be a car driver and have a driving licence.

Key Tasks & Responsibilities:

·         To triage assess plan and implement and evaluate all aspects of patient care.

·         Develop care plans to ensure continuity of treatment.

·         To work integrated within the neighbourhood team, to ensure holistic care is provided.

1.         Communication:

You shall be required to communicate complex and sensitive information to patients and carers. Use tactful and persuasive skills to gain the patients co-operation in their care management plan. Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated. Maintain skills with regards to latest technology Participate in multi-disciplinary/multi-agency meetings as appropriate, e.g., Gold Standard Framework. You shall be required to gain consent and complete mental capacity assessments at each intervention. 

2.         Analytical and judgemental:

You shall be required to ensure own workload is planned and prioritised according to patient needs. Once completed triage induction you shall ensure a thorough triage of referrals to ensure the referral is suitable for the service. You shall allocate the work for the team ensuring safe delegation of care plans.

3.         Planning and organisational Skills:

As an autonomous worker you shall be required to ensure patient held records are completed for each visit, and that SystmOne inputting is completed daily. You shall be required to manage your daily route and manage your time independently

Essential criteria
  • Ability to travel to all rural locations to carry out the post.

Education and qualifications

Essential criteria
  • Current NMC registration
  • RN Qualification (Part 1 of register) or equivalent.
Desirable criteria
  • Leg Ulcer Management
  • IV Administration /cannulation
  • ENB 998/mentorship preparation or equivalent
  • Mentorship qualification is essential for the post, but not essential at interview.

Experience and knowledge

  • Good communication skills which demonstrate tact, diplomacy, empathy, and sensitivity
  • Able to communicate effectively
  • Able to provide high standards of care.
  • Ability to manage stressful situations.
