We have an exciting opportunity for a highly skilled Occupational Therapist to join our South East, Home First, Discharge to Assess (D2A) Teams based in Tamworth.

We want you to work for us in helping our Allied Health Professional (AHP) family to assess and treat our communities with reablement and rehabilitation at the centre. You will work with our communities and individuals who are on the Home First Discharge to Assess (D2A) pathway following discharge from hospital.

We are looking for someone who is positive, enthusiastic and innovative with the ability to demonstrate excellent clinical, communication and organisational skills. Providing assessments and goal setting to promote and maximise people’s independence to enable them to remain in the community for as long as possible.

You will work as part of a well-established, friendly and supportive integrated multi-disciplinary team of Occupational Therapists, Physiotherapist, Therapy Assistant Practitioners, Social Care Practitioners, Home First Support Workers and administrators.

The focus of this role is to support the Home First (D2A)/Reablement Service and demonstrate a real focus on rehabilitation to support the individuals you see to maximise their potential, improve their independence and reduce the need for their reliance on long term care and support.

As part of this role you will be expected to have:

·       To assess new service user referrals and care/support needs. Assesses service users and formulates, develops, implements and evaluates programmes of treatment /care/support for individuals, families and carers.

·       Extensive knowledge and skills in completing manual handling assessments, risk assessments and provision of appropriate equipment.

·       To use professional knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider each service user’s personal circumstances and preferences in order to implement and adjust programmes of care and intervention.

·       To establish and maintain effective working relationships both within the Partnership Trust and with external organisation in order to meet service requirements and support and demonstrate the Trust’s equality values.

Qualifications and Training

Experience

  • Relevant experience to the role including assessment and interventions for a wide range of disabilities.
  • Ability to manage time and caseload effectively.
  • Supporting service users, families and carers in dealing with the impact of disability and life limiting conditions
  • Working in a multidisciplinary team.

  • Experience of using standardised assessments and outcome measures
  • Experience of negotiation and conflict management in clinical situations.

Skills, Knowledge and Abilities

  • Evidence of specialist clinical knowledge relevant to the post.
  • Ability to demonstrate an in depth knowledge of a variety of medical conditions and disease processes, equipment and minor home adaptations.
  • Understanding of Information Governance, Data Protection, HCPC /COT standards and Safeguarding Vulnerable Adults.
  • Evidence of specialist knowledge in Moving & Handling Assessment

  • Knowledge of relevant NSF’s and NICE guidance. Understanding of wider NHS agenda and its application to the local context.

  • Current valid driving licence and ability to travel in a timely manner withing geographical area.

  • Flexibility with working hours to enable 7 day working.
