Job overview

This is an exciting opportunity to work as part of the expanding Health and Justice Reconnect and Outreach Service in Sussex. 

You will be responsible for working in Health & Justice Reconnect and Outreach service within Sussex. Health & Justice is not itself a treatment service, but it is an identification, assessment, and engagement service to support people to access and engage in services to address their identified vulnerabilities and health needs.

You may be required to complete visits within the local prison as part of the engagement work. Additional vetting may be applied for by the Trust as part of the recruitment process however this can be discussed further dependent on individual need.

You will be responsible for providing support to vulnerable individuals upon leaving Custody, Court, or Prison, where further support is identified as being required. 

You will be responsible for working closely with the individual to prioritise need and plan actions to address identified need.  This may include supporting an individual to make a referral to a specific service and support the individual to attend the initial appointment with that service.

Due to the nature of the role you will be required to have a driving licence and access to a car with business insurance as transporting of service users may be required.

Main duties of the job

Working with service users referred to Health & Justice and the individual’s carers, helping to promote self-determination, choice and autonomy.

  • To effectively establish a therapeutic relationship with service users and relatives / carer.
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Assess patient's individual care needs, agree, and refer on to appropriate agency. Involve the individual in the planning and prioritisation of their activity where possible. Provide short term intervention to encourage individual to engage with services and attending their initial appointments.
  • Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition
  • Utilise agreed outcome measures to review the effectiveness of process, as part of the Health & Justice Service.
  • Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
  • Assist in ensuring that the aims and objectives of the service (as set by the Team Leader) are fulfilled and to identify factors which may inhibit these from being achieve.

In addition to the responsibilities already listed -

 Systems and equipment

  • Maintain timely data collection and regular inputting of information through the use of data collection systems e.g. daily diaries/RIO, Safeguard, ensuring that confidentiality is maintained at all times in accordance with legislation and Trust policy.
  • To work in partnership with other service providers to access and provide equipment and opportunities for service users to maximise functioning where possible.
  • Responsible for ensuring actions are carried out where safety alerts are issued regarding equipment. 

Decisions and judgements

  • Work closely with colleagues within Health & Justice Service.
  • Ensure that practice is evidence-based paying particular attention to the Trusts guidelines, policies, protocols and pathways.
  • Be available for individual supervision with agreed clinical supervisor in line with Trust policies.
  • Implement, review and maintain Trust Policies and Procedures and propose changes to working practices within the team.

Person specification

QUALIFICATIONS & TRAINING

Essential criteria
  • Competent in literacy and numeracy.
  • Basic knowledge of physical / mental condition and rehabilitation.
  • Computer Literacy
Desirable criteria
  • NVQ Level 2 Health & Social Care or equivalent of.
  • Training in Supporting / Mentoring
  • Adult / Child Safeguarding Training

EXPERIENCE

Essential criteria
  • Experience of working with a range of vulnerabilities which may include mental health, physical health, learning disabilities, abuse, addiction, and social issues.
  • Experience of working with a range of statutory, non-statutory agencies.
Desirable criteria
  • Experience and/or sound knowledge of the Criminal Justice System.
  • Lived Experience of vulnerability, criminal justice systems and / or leaving care.
  • Experience of working in a community-based setting.

SKILLS, KNOWLEDGE & ABILITIES

Essential criteria
  • Awareness of a range of vulnerabilities. An awareness of the impact of vulnerabilities on individuals.
  • Basic understanding of the Recovery Model.
  • Ability to form rapport with individuals and groups.
Desirable criteria
  • Knowledge of relational security.
  • Aware of local community-based services.
