Specialist Nurse – Care Home Intensive Support Team

Job overview
We have an exciting opportunity for an enthusiastic and motivated
Band 6 Nurse who is registered with the Nursing and Midwifery Council (NMC), to join our innovative Care Home Intensive Support Team (CHIST).
CHIST was formed in May 2020 in response to the Covid-19 pandemic and has continued to evolve as the team has become more established whilst working in conjunction with Staffordshire County Council, Integrated Care Board (ICB), Care Quality Commission (CQC) and Registered Providers. CHIST are working collaboratively with Staffordshire and Stoke-on-Trent Integrated Care Board, providing Nursing and Allied Health Professionals (AHP) staff for targeted, time limited support to specified care homes as prioritised through local system escalation processes.
The role is community-based with remote working and will require you to work county wide covering care homes registered with Staffordshire County Council to meet the demands of the service. You will require suitable access to transport to work across the geographical area covered.
Main duties of the job
As part of this role you will be expected to assess residents care/support needs when required, providing advice and support whilst working closely with the care home staff to formulate an appropriate plan of care for resident as well as undertake tasks concerning the day-to-day organisation and operation of the team, working with the team leaders as required.
You will have excellent interpersonal and communication skills and an ability to quickly develop effective relationships with care home staff.
You will also hold:
· specialist clinical skills in a wide range of areas to enhance the health and wellbeing of care home residents providing hands on clinical support, advice and guidance based on resident’s needs. This could include support and advice with; moving and handling, falls management, promoting meaningful activity, personalised care planning, nutritional advice/signposting, and navigating links with other Health and Social Care Services.
· To maintain and develop skills and knowledge to deliver a range of interventions (including physical interventions, training and support to care home staff, onward referrals for service users) and sets an example of good practice to other team members including skill sharing.
· You will be skilled in risk management and demonstrate clinical leadership skills with an ability to prioritise the support needed in a care home, and escalate any issues appropriately.

QUALIFICATIONS & TRAINING
Essential criteria
- Degree in nursing or equivalent
- Registration with NMC
- Post Graduate studies in relevant nursing field such as; end of life care, long term conditions/palliative care or related/relevant subject
- Mentorship qualification (998-997)
- Post qualification courses in nursing
- Attendance at relevant networks e.g. Special Interest Groups
- Up to date field work/pre-registration student training.
Desirable criteria
- Modules at Masters level
- Evidence of further clinical study
Experience
Essential criteria
- Relevant experience to the role including knowledge and management of long term conditions
- Ability to manage time and workload effectively
- Working in a multidisciplinary team
Desirable criteria
- Experience of negotiation and conflict management in clinical situations
- Experience in leading and supporting staff
- Experience of involvement in service improvement
