Job overview

An exciting opportunity has arisen to appoint a full time substantive Band 5 Occupational Therapist to provide support for service users with learning disabilities and highly complex needs in a community setting within an Intensive Support Service. The hours are 9am-5pm Monday to Friday (Flexible working will also be considered)

 

The successful applicant will be part of a fast paced crisis response team and so we are seeking a highly motivated, organised and skilled Occupational Therapist, preferably with a background or previous experience in Learning Disabilities. An important part of the role will be to work as part of a multi-disciplinary team and support the delivery of an integrated ‘whole systems’ approach for people with complex health needs.

 

An essential element of the role will be to maintain and promote high standards of practice and high quality outcomes, “Advocacy, Enablement and Empowerment” is the focus of our work. Applicants will need to possess the necessary drive, enthusiasm and commitment to deliver person centred outcomes in a time limited framework, closely working in partnership with service users and their families/carers. 

Main duties of the job

North Staffordshire Combined Healthcare NHS Trust aims to provide accessible, non-stigmatising local services that contribute to the assessment, intervention, treatment, rehabilitation and enablement of people with learning disabilities, which promotes the prevention, early detection and treatment of health related issues for adults with learning disabilities in the community. The service is wholly committed to the use of Person Centred Planning approaches and affording adults with learning disabilities opportunities to maximize full citizenship, choice and control. The Occupational Therapist has a lead role in promoting, enhancing and maintaining the health and wellbeing of adults with learning disabilities.

Detailed job description and main responsibilities

       Under the supervision of the Band 6 Occupational Therapist and Team Manager as appropriate, work across a shared caseload that reflects the priorities of the local service by ensuring the assessment of therapy needs, particularly in relation to:

 

       Using specialist knowledge to independently undertake assessments and evaluate interventions with clients with diverse environmental, social, physical and mental health needs; adapting approach to the varying level of learning disability.

 

       Close working with other services aiming to reduce health inequalities and improve access to universal services.

 

       Opportunities to support access to mainstream health services for adults with learning disabilities.

 

       The provision of timely intervention aimed at promoting and maintaining the health of adults with a learning disability, through the implementation of therapeutic interventions.

 

       Provide assessment, training and advice on lifestyle changes and adaptations to the client’s physical and social environment for the client and carers.

 

       Will undertake a variety of Occupational Therapy assessments to assist in the identification of person centred support needs and intervention.

 

       Plan, organise and implement a range of treatments aimed at enabling clients to achieve their maximum potential in activities of daily living.

 

       Provide a wide range of therapeutic interventions ensuring evidence based practice according to the National Institute for Clinical Excellence (NICE) guidelines to achieve therapeutic goals.

 

       Demonstrate the clear and effective use of outcome measures for all clients to ensure that optimum physical and mental health needs are met / maintained. 

 

       Establish and sustain links with primary care settings and non-statutory agencies facilitating access to universal services for adults with learning disabilities.

 

       Where appropriate contribute to assessments of adults with learning disabilities eligible for Continuing Healthcare funding using the Decision Support Tool and contribute to the coordination of support packages ensuring person centeredness, high quality standards and value for money  to meet the desired clinical outcomes.

 

       Demonstrate the ability to assess plan and provide a strategy for supporting clients with mental health needs in addition to their learning disability, including the adoption of assessments through collaborative working, embracing the process of the Care Programme Approach, (CPA).

 

       Work face to face with individuals who might display physical aggression and who are in psychological distress.

 

       Deliver services in a sensitive, respectful and person centred way adapting systems of communication to meet service user’s cognitive ability.

 

       Keep abreast of developments in current best practice, trends, issues and research in learning disabilities and related topics in line with the divisional business plan, and national occupational therapy practice within services for people with a learning disability. 

 

       To take responsibility for own Continuous Professional Development (CPD) and facilitate and encourage CPD for those staff for whom they are responsible.

 

       Maintain up to date and accurate individual client records in line with Trust policy and the procedures of the Intensive Support Team using the agreed IT systems to produce high quality reports as appropriate and to maintain the highest standard of care.

 

       To ensure that all policies, procedures and guidelines are followed, reporting any deviations to senior staff. 

 

       To set professional standards of care and to monitor measure and audit the effectiveness of services provided.

 

       To undertake any other duties commensurate with the post as directed by the relevant managers and as identified as service priority.

 

       To comply with the College of Occupational Therapist’s Code of Ethics and Professional Conduct, Health and Care Professional Standards of Proficiency for Occupational Therapists and local policies and procedures.

Qualifications

Essential criteria
  • Appropriate professional qualification – Diploma / Degree in Occupational Therapy. Hold Health and Care Professional Standards of Proficiency for Occupational Therapists (HCPC) Registration.

Experience

Essential criteria
  • Experience of working in clinical practice either as an Occupational Therapist, Occupational Therapy Technician or Trainee Occupational Therapist.
