Main duties of the job

To project manage the remote monitoring tools that are currently used across the Virtual Wards (VW) – Docobo, MySense, Oxy@Home/FLO and any new remote monitoring tools that may be used going forward.

Working with the VW Matron on new/existing remote monitoring pathways to maximise utilisation of the additional licences/kits – both Acute and Sub Acute Wards.

Performance metrics up to 30% of Acute VW patients are remotely using a digital solution, up to 50% of the Specialised Pathways, 70% of the Sub Acute.

Highlight Reports for the Acute Care at Home Board, Remote Monitoring Board and UEC Board.

Supporting Acute Care at Home Service with the replacement of Docobo (current contract is only until Feb 2025 by linking into meetings, migration (Doc@Home Migration to MS Azure + FAQ UK-34 STW & SSOT).

Capturing any learning from other Trusts that the ICB would find beneficial and arrange visits to understand scope and implementation; supporting the learning and evaluation of Docobo.

Utilising the use of remote monitoring / Docobo in Care Homes – this post or AC@H couldn’t take on the full rollout as ultimately it links into primary care with escalation to CRIS 2hr response, but could be our support to get Darwin Court and any other over the line before the contract is up.

IG and due diligence goes hand in hand with remote monitoring tools.
