Discharge Facilitator – Band 3 - Ward 222 - NIVCCU

An exciting opportunity has arisen for enthusiastic, highly motivated individual to join our team on Ward 113 - Respiratory at Royal Stoke University Hospital as a Discharge Facilitator.

 

Main duties of the job

Main duties of the job

You will assist the qualified nurses to plan and facilitate timely discharge processes for patients from point of admission to the ward environment to the discharge of patients to their own home or alternative care facilities.

The post holder will have excellent communication skills and have the ability, drive and enthusiasm to work within a motivated team to provide high quality patient care  the post holder will ideally have experience as a Discharge facilitator or   Skills
