**INTERNAL MPFT APPLICATIONS ONLY**
The post holder will be a Registered Nurse for Adults with Learning Disabilities (RNLD) employed by Midlands Partnership NHS Foundation Trust, who will work in partnership with the Acute hospitals i.e. County Hospital, Stafford and Queens Hospital Burton alongside the Staffordshire Community Teams.
They are called the Acute Liaison Nurse and their role is to support the person with a learning disability, their carers and the acute staff.
They also provide advice, support and awareness training to acute services staff.
Main duties of the job
• Pre-admission planning
• Carer support and liaison
• Pre discharge planning
• Post discharge follow up call.
• Facilitation of Outpatient appointments
• Ensuring ‘reasonable adjustments’ are made where necessary
• Advice for acute staff around issues such as: communication, risk management, consent to treatment and the importance of involving and listening to significant others for pertinent information.
• Service user education through the use of “Easy Read” leaflets
• Guidance on Reasonable Adjustments

Detailed job description and main responsibilities
To be responsible for community or hospital admissions, including liaison with other agencies, service users and carers.
To evaluate the outcome of nursing interventions to ensure that care plan objectives have been met and that health gains have been achieved.
To co-ordinate care with the MDT and hospital staff at points of attending appointments, admission and discharge.
To provide advice, practical support and guidance with regards to people who may lack capacity to make decisions about their health.

- Previous Band 6 experience.
