Job overview

We are looking for a passionate qualified Level 1 Social Worker to join our Adult Social Care team within the Newcastle District. We are a community based team within the older persons/ physical disability team working with Adults over 18 years of age with a range of physical and mental health needs.

We are a supportive and dynamic organisation and provides opportunities to progress and develop within your Social work career. We are delighted to offer a specialist Social Work Learning Academy (SWLA) which is an internal academy within our organisation which offers ongoing Social Work training and Continued Professional Development.

On appointment, you will be enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework.  The relevant SWLA ‘stage’ (i.e. stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

Main duties of the job

As a registered social workers you will have a key role to play in relation to supporting the delivery both the personalisation and social inclusion agendas ensuring quality public facing services including health and social care services.

As part of this role you will be expected to:

·       carry out a range of assessments including holistic “needs” led assessments, care and support planning and reviews, and, where appropriate, initiate a multi-disciplinary assessment

·       Be an integrated and supportive member of a team, planning, actioning, monitoring and evaluating work with the supervision of an experienced Social Worker and the Advanced Social Work Practitioner lead.

·       Be responsible for working collaboratively with other Trust services to provide seamless care for adults/carers on defined care and support pathways with particular emphasis on ensuring safe and effective transitions.

Participating on the duty/ACCESS or priority rota, to carry out a range of functions including screening/ carrying out an assessment for health and social care services for adults and carers utilising a range of assessment tools including a Risk Screening Tool.

Qualifications & Training

Essential criteria
  • Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body
  • Ensuring current registration with the Professional Social Work regulatory body at all times
  • A commitment to on-going personal and professional development
  • To undergo appropriate training to achieve Assessed Year in Practice (ASYE)
  • Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint
  • A clear understanding of and commitment to equal opportunities and anti-discriminatory practice

Experience

Essential criteria
  • Experience of working with Adults and older Adults with mental health needs and their carers
Desirable criteria
  • Experience of social work in other user groups/settings i.e. Children and Families, Learning Disability, Mental Health
