Job overview

Staffordshire and Stoke-on-Trent Integrated Care Board (ICB) was created on 1 July 2022 and is responsible for planning and commissioning high quality health services for the people of Staffordshire and Stoke-on-Trent.  

 We have strong clinical leadership and work as part of a complex system of health and social care, which includes Local Authorities, Providers, the Independent Sector, Staffordshire Training Hub and GP practices.

 We are looking for a Practice Nurse Facilitator for the North Staffordshire & Stoke on Trent area, to join the team to work alongside three other Practice Nurse Facilitators across the ICB.

 Band:      AFC Band 7

Hours:    1 day per week (7.5 hours per day) (flexible working arrangements available)

Location:       Home Based and home working (majority of posts).   However, there may be occasions when you will be required to attend a location hub/practice; usually this will be pre-planned in advance, and with a days’ notice.

Main duties of the job

 

An exciting opportunity has arisen for an experienced highly motivated individual to join the Primary Care Team working across the Staffordshire and Stoke-on-Trent Clinical Commissioning Groups.

This is an exciting and innovative role, for a forward-thinking individual who will provide professional advice and leadership to General Practice Nurses incorporating clinical service change and improvement.  A key focus will be on strengthening the nursing workforce by identification of educational needs and providing access to training and development opportunities needed to implement high quality and evidence-based practice. The role will act as a conduit between Practice Nurses, Nurse Associates, pre-registration nurses and Health Care Assistants.

The Staffordshire and Stoke-on-Trent NHS Integrated Care Board (ICB) is responsible for planning functions; managing the NHS budget and arranging the provision of health services in the geographical area.

 Location          Home based/hybrid working (most of posts).  

Travel on occasions to a Hub location with at least 1 days notice

 New starters are expected to collect IT equipment, from offices in Stoke-on-Trent, and if based at home are required to have appropriate desk/chair/ relevant equipment in compliance with DSE regulations

Detailed job description and main responsibilities

 We are looking for a nurse who is highly motivated, enthusiastic and enjoys a challenge. In addition, you must hold current registration and have a sound understanding of the role of the nurse in Primary Care. The ideal candidate will be a highly experienced nurse and have a minimum of 5 years’ experience in General Practice and ideally already working within the Staffordshire and Stoke on Trent ICB area. Experience

Essential criteria
  • Evidence of building positive relationships, engaging and collaborating effectively with others internally and externally
  • Excellent IT experience including advanced use of internet and Microsoft packages
  • Experience of undertaking patient surveys/audits
Desirable criteria
  • Extensive experience of working in a similar role within the Staffordshire and Stoke-on-Trent area
  • Experience of developing and delivering training sessions at all scales

Qualifications

Essential criteria
  • The post holder is to hold a current registration with relevant professional body and to maintain registration whilst in post
  • The post holder is expected to hold a recognised nursing qualification and will be expected to work within the NMC Code of Professional Conduct and to revalidation standards
  • Registered first level nurse or above
  • Masters/post-graduate education or similar equivalent academic ability
  • Evidence of continuing professional development, which demonstrates the theory and practice to operate at a senior level.
Desirable criteria
  • Independent and/or supplementary nurse prescribing qualification

  • Highly developed and accurate report writing skills to present data and information to a variety of internal and external audiences

  • Good negotiation and conflict management skills
Desirable criteria
  • A good understanding and knowledge of clinical commissioning and national policy direction relevant to general practice
  • Ability to demonstrate a learning and educational culture
