The direct provision of high-quality evidence based person-centred physical and mental health care to patients within a community pathway within the Mental Health Function of the Children and families care Group.
Prif ddyletswyddau'r swydd
Providing leadership in the delivery of high quality, safe and effective care to the wider multi-disciplinary team.
Facilitating learning through the development and delivery of specialist clinically focussed education and training programmes.
Receive CYP (and families) with undifferentiated and undiagnosed problems and make an assessment of their health care needs, based on highly developed nursing knowledge and skills, including physical examination skills.
Order necessary investigations, and provide treatment and care both individually, as part of a team, and through liaison with / referral to other agencies, e.g. ECGs, Blood tests etc.
Plan, develop and manage specialist packages of care, employing frameworks such as the Care Programme Approach (CPA), CETR, Child Protection, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use.
Prepare reports encompassing medical and advanced nurse practitioner interventions in readiness for review and care planning meetings.
Undertake health promotion and prevention activities including, for example, educating CYP/Families and groups in the side effects of medication and lifestyle choices.
Working with education providers to develop nursing programmes/training.
Provide nursing leadership for nursing staff within the team and the wider CYPMH as needed

Qualifications and Training

- RMN or RN (Mental Health).
- Masters Level qualification in health/social care or demonstrable equivalent experience
- Therapeutic psychological interventions qualification
- Mentorship in healthcare setting or equivalent - with experience of supporting student nurses and other professions

- Training in Venepuncture and ECG recordings (post holder will be required to complete this training in addition, if not already held)
- Advanced physical healthcare assessment qualification
- Independent and supplementary prescribing qualification
Experience

- Experiencing of working effectively at a senior nursing level in a multi-disciplinary CAMHS team
- Experience of working with a range of statutory and non-statutory agencies
- Experience of engaging in service development activities
- Evidence of promoting research, evidence-based practice, clinical excellence, and quality approaches to support outcome focussed care
- Experience in facilitating learning within clinical practice
- Experience of managing, supervising and supporting nursing teams
- Experience of the administration and management of medicines according to policy and local protocols
- Experience medicines optimisation with young people and families as part of their care
- Experience in contribution to the development of clinical service provision

- Relevant knowledge and practice of local and national safeguarding procedures and guidelines
- Understanding and application of local CAMHS policies and procedures
- Knowledge of the principles of Care-Coordination /CETR and Safeguarding Children framework
- Skills and knowledge in the inequalities in health, motivated to bring about change for these groups
- Skills in writing and presenting reports
- Knowledge and application or relevant legislation and guidance including the NHS Long Term Plan, Mental Capacity Act 2005, The Mental Health Act 1983 (as amended), The Community Care Act 1990, The Children's Act, The National Service Framework for Children and Future in Mind and relevant Clinical Governance
- Extensive and robust skills in assessing, planning and evaluating mental health care/treatment interventions, including those with multiple/complex conditions
- Expertise in psychosocial interventions, including preventing and managing crisis presentations
- Skills in interacting therapeutically with CTP who have a range of presenting problems, including where engagement with healthcare services have been difficutlt
- Skills in managing the physical as well as mental healthcare needs of the patient group
- Skills in co-ordinating complex care packages for those with complex needs including mental health and Neurodevelopmental needs
- An ability to coordinate and deliver coaching programs and skills in teaching and assessing
- An ability to co-ordinate, develop and deliver a program of health promotion interventions to patients
- An ability to work independently and within a team
- An ability to use the internet, email and basic work processing skills
- An ability to deal with challenging situations
