As a registered professional do you have capacity to engage in work for 2 days per week or more?  Join NHS Midlands and Lancashire as a bank nurse, working within our continuing healthcare department to conduct assessments and reviews of patient care. 

Registered Nurse/Social Worker/Registered Allied Health Professional? Interested in a new challenge using your excellent assessment skills?  We are looking for qualified Senior Practitioners to join the Personalised Healthcare Commissioning (PHC) service to work with our Teams in supporting ICB`s in providing support for patients in receipt of health funding.

The post holder will play a key role in supporting the Personalised Healthcare Commissioning service across the region. Working within the remit of the National Framework for Continuing Healthcare and other NHS funding streams, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.

 You should be well versed in dealing with complex and sometimes contentious information.  

The Post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.

Main duties of the job

The postholder will be expected to review health funded care packages across the responsible integrated Care Board (ICB) area.

 

You will support colleagues in relation to NHS Continuing Healthcare Funding ensuring current practice is being embedded into clinical practice, promoting best practice and compliance with the Department of health processes.

 

The role will be responsive to ICB priorities and expectations whilst working on their behalf and representing them through work with NHS Continuing Healthcare.

Job responsibilities

Communication and Relationship Skills 

·         Communicating information, risks, issues and dependencies, including briefings and reports 

·         Maintaining constructive relationships with a broad range of internal and external stakeholders 

·         Working closely with care providers involved in delivery of the service 

 

Knowledge, training and Experience 

·         Providing coordination of, and participating in, meetings and providing information, advice and support as required 

·         Assisting and supporting individuals and representatives in relation to the Personalised Healthcare process 

·         Maintaining up-to-date knowledge of legislation, policies and guidance 

·         Providing relevant and timely specialist advice and guidance on functional and information matters relating to the service 

 

Analytical and judgement skills 

·         Supporting investigations into an individual’s eligibility for NHS Continuing Healthcare following an individual challenge or dispute 

·         Working with members of the team to investigate the causes of any variance from target/plan and proactively contribute to the implementation of solutions 

 

Planning and organisational skills 

·         Contribute to maintenance and monitoring of service standards around quality 

·         Supporting the Personalised Healthcare team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery 

 

Physical skills 

·         Keyboard skills 

·         Access to vehicle for occasional travel between sites 

 

Patient/Client Care 

·         Managing a caseload of patients  

·         Ensure the principles and duties of safeguarding are holistically and consistently applied  

·         Supporting the arrangement of services required to meet the needs of individuals who qualify for NHS Continuing Healthcare and for the healthcare element of a joint care package 

·         Fostering transparency and a person-centered approach with patients, families and representatives 

·         Ensure the principles and duties of safeguarding are holistically, consistently and contentiously applied  

 

Policy/Service Development 

·         Working within a team of Personalised Healthcare Assessors, ensuring a high-quality service is delivered, supporting and mentoring and supporting staff to achieve continuous improvement. 

·         Ensuring Personalised Healthcare guidance is followed and services are delivered to maximise patient safety 

·         Actively supporting the Personalised Healthcare team in working towards goals and objectives 

·         Contribute to maintenance and monitoring of service standards around quality  

·         Comply with NHS Codes of Conduct and Standard Business Conduct and/or NHS Trust Standards of Business Conduct policies 

 

Finance and Physical Resources 

·         Knowledge of Personal Health Budget process 

 

Human Resources/Staff Management 

·         Managing staff in line with regional and local HR policies 

·         Mentoring and supervision of students 

 

Information/Resources 

·         Providing accurate and legible recording of information 

·         Ensuring data systems are kept up to date and maintained in line with standard operating policy and procedures 

 

Research and Development  

·         Participate in relevant internal surveys 

 

Freedom to Act 

·         Comply with NHS Codes of Conduct and Standard Business Conduct and/or NHS Trust Standards of Business Conduct policies 

·         Assisting in the facilitating of multi-disciplinary assessments and patient reviews in line with standard operating procedures 

·         Supporting the Senior Practitioner to ensure that all processes implemented are compliant with the NHS Continuing Healthcare National Framework  

·         Assisting in the management of cases as directed by the Senior Practitioner and the Team Lead 

·         Delivering against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines 

·         The post holder will be responsible for ensuring their mandatory training is up-to-date
