Due to promotion within the team an exciting opportunity has arisen for a Registered Nurse to join a dynamic team of nurses that provide commissioning services for people with complex care needs who are eligible for NHS Continuing Healthcare funding or NHS-funded nursing care.

The candidate should have knowledge of the NHS Framework for Continuing Healthcare.
The successful candidate will provide specialist nursing assessment and review for adults and older people presenting with long-term care needs who may be eligible for NHS Continuing Healthcare, NHS-funded nursing care and jointly funded packages of care in line with the NHS Framework for Continuing Healthcare.
Experience of undertaking assessments, and working with patients, families and carers is essential. The CHC team are responsible for the assessment and review of people who reside in nursing care homes and the clinical commissioning of packages of care for people with continuing healthcare needs who reside in their own homes.

Assist in the implementation of policies involving NHS Funded Nursing Care and NHS Continuing Health Care in Care Homes
To review eligibility for NHS Funded Nursing Care and Continuing NHS Care
To undertake Nursing Assessments for patients with identified health needs
To make recommendations following Nursing Assessment relating to all identified needs of a patient
To trigger referral for a multidisciplinary team assessment for those clients whose physical and mental health care needs require consideration for Continuing NHS Care funding
Through assessment, identify clients who are at risk and act / manage any issues related to the findings
To report any concerns related to client care following assessment to the CHC Lead Nurse within the Continuing Care Team, Care quality commission (CQC) and Adult Protection and Social Services
To maintain robust Nursing Assessments/Reviews and care planning records
To identify specialist nursing equipment needs and advise about provision of that which is over and above the minimum standard. To ensure suitability of equipment for patients with identified health problems
To promote physical and mental well-being and maximise client and carer empowerment in the management of their care, and where necessary ensure individuals have access to an advocate
To undertake Joint Assessments with other professionals from within health and social care, to ensure the health needs of patients are identified and met appropriately
To communicate outcomes of NHS Funded Nursing Care / Continuing NHS Health Care Assessments to Continuing Care Leads and Continuing Care Review Panel
To act as an information resource for Leicester, Leicestershire and Rutland CCG in relation to Free Nursing and Continuing NHS Healthcare, providing appropriate advice to the public, professionals, and others as appropriate
To undertake retrospective assessments when necessary in conjunction with other professionals for health and social care and develop needs portrayals
To work with the client, family, NHS and Independent Providers to identify options available to provide the appropriate care provision for the client and present to the Panel for funding approval
Possess a working knowledge of the Department of Health guidance on NHS Funded Nursing Care / Continuing NHS Health Care and Ombudsman and maintain relevant and up to date knowledge and skills
To act in accordance with the legal requirements and statutory rules related to practice and registration, NMC Code of Conduct and PCT Policies and Procedures including Financial Procedures, Health & Safety Policies and Safe Systems of Work
To attend all mandatory updates
To promote clinical governance within all settings, encouraging the development and participation in clinical audit, research evidenced based practice and reflective practice and life long learning
