Support Time Recovery worker: Reconnect /Outreach

Are you a Community Support Worker looking for a genuinely rare opportunity to work with prison leavers both within Prison and in the community? If so, you're in luck.

The STR worker in Reconnect will be working, following vetting, within HMP Hewell and HMP Stoke Heath and supporting clients in the community. Mainly in the Worcestershire/ Herefordshire area , but to include all West Mercia.

RECONNECT seeks to improve the continuity of care of vulnerable people leaving prison by working with them before they leave and support their transition to community-based services, thereby safeguarding health gains made whilst in prison. Midlands Partnership University NHS Foundation Trust has an opportunity to join our Reconnect team in West Mercia.

As an experienced Support Worker from the NHS or private sector, you'll join a team that works in close partnership with HMPP services, and you'll join a Trust that puts people at the heart of what we do.

You must be able to travel to various locations for your shifts as the role requires travelling over the West Mercia area. Expenses for travel are paid.

RECONNECT seeks to improve the continuity of care of vulnerable people leaving prison by working with them before they leave and support their transition to community-based services, thereby safeguarding health gains made whilst in prison.

As a Support Time Worker you will engage with individuals up to 12 weeks before release from custody working alongside and supported by a qualified practitioner. You will continue to provide support and guidance under supervision from the qualified practitioner up to 6 months post release, using your own skills and developing knowledge to assist the individual in achieving their established goals.

In summary, these are the things that you will have:

  • NVQ Level 3 in (Mental) Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSEs or functional skills)

or Good knowledge of Mental Health and Social Care and relevant experience

  • Experience of working with people with Mental Health Needs (non-Dementia related)
  • Strong team-work skills
  • A high level of self-motivation and a flexible approach
  • A positive attitude and be naturally caring and compassionate
  • A commitment to high-quality care and values and evidence-based practice
  • Excellent teamwork skills, and professionalis
