An opportunity has arisen for a Band 6 Community Mental Health Nurse to join our dynamic and friendly Integrated Mental health Team based in Codsall, providing care for the local community and surrounding areas. The team in Codsall is multidisciplinary and provides high quality care for service users that experience mental health conditions. The team is part of the wider Integrated Mental Health Team in Stafford & Seisdon and links together for CPD events, supervision and partnership working. 

We are seeking an experienced Mental Health Nurse who has a keen interest in working with this client group to provide a holistic overview and lead on the care and treatment of the person’s journey through to recovery. The successful candidate will provide nursing leadership with the MDT, support junior colleagues in their development and facilitate student nurses in their training.

• Working in an identified adult community mental health pathway, with the flexibility of working across pathways, dependent on skill and service need.
• Delivering a high quality clinical service to patients and their families.
  Managing a complex caseload and responsible for clinical and caseload supervision of other team members where identified.
• You will be responsible for undertaking clinical assessments, in accordance with the clinical pathway, in order to establish the person’s needs, goals, risks and strengths in order to generate a formulation based care plan agreed with all involved.
