Job overview

We are offering you an exciting opportunity to join our Adult Community Nursing Service in the Burton-upon-Trent area as a NMC registered community staff Nurse.

We pride ourselves on our strong leadership, innovation, prevention and implementation of the care within the living model in all our patient care offering an innovative, integrated community service providing care to patients in their own homes, care homes and working closely with our GP practices.

We work closely across the Nursing teams, AHP’s including Podiatry, Physio, Falls Team, Stroke and Rehab team including our out of hours service, to ensure that we work collaboratively to deliver our care across all the professions, recognising the importance for patient centred approach.

Previous experience in working in the community is not necessary, however, you do need to be able to demonstrate a commitment to delivering high quality targeted personalised care. We actively support the development and learning of all staff and you will be well supported in your role priding ourselves on growing our own staff and the leaders of the future.

Due to the nature of the role you will be required to have suitable transport to cover a large geographical area.

You will be required to work flexibly on a rota system across a 7 day working week, however; we will consider request for flexible working around the requirements of the service.

Main duties of the job

As a Community Staff Nurse you will be expected to deliver a high quality service, working closely with GP’s and multi-disciplinary teams to provide a range of care across the spectrum of health conditions, and preventing avoidable hospital admissions.

You will work independently in a community environment and as part of your role you will be expected to participate in the:

·      Assessment of new service users, referrals and care/support needs.

·      Assess service users and formulates, develops, implements and evaluates programmes of treatment /care/support for individuals, families and carers.

·      The care/support plans may include the following:

o   Promoting independence/self-management and maintaining safety.

o   Increased functional ability

o   Improvement in health and wellbeing including mental health

o   Interventions in disease/condition management

o   Prevention and reducing of health inequalities

o   Admission avoidance / Early supported discharge

o   Palliative and end of life care

o   Undertake Tissue Viability Risk Assessments

o   Bladder, catheter and bowel care
