Job overview

As a member of the Primary Care Network Mental Health Team the post holder will provide high quality nursing support, assisting in the Dementia Review, Medicine Management, Severe Mental Illness (SMI) Health Check pathways, as well as other specialist mental health services.

The Nurse Associate Practitioner works under the supervision of a Registered Nurse to assist in the assessment, care delivery and support of patients and carers and to provide support and education to relevant professionals and colleagues.

80% of the post holder’s time will be spent in direct and indirect care with patients / carers and in providing support and education to relevant professionals and colleagues.

15% of the post holder’s time will be sent in education, training and professional development.

5% of the post holder’s time is spent in admin and clerical duties.

Main duties of the job

  • Participate in the assessment process by conducting initial interviews with patients, carers and professionals involved in the patient’s care and treatment.
  • Develop trusting relationships with patients and carers to enable home visits.
  • To monitor the patient’s physical health by conducting observations (temperature, pulse and blood pressure) and taking blood samples via venepuncture if requested by the GP.
  • Liaise with patient’s GP, family, Care Manager, other multidisciplinary teams and Consultant Psychiatrist regarding the patient’s health and well-being
  • To provide support, advice and information to the patient and carers.  This may include brief counselling using empathy and reassurance
  • To communicate in writing to the GP regarding the Team’s intervention with a patient when they are discharged.
  • Participate in the In-Service training programme for all staff.
  • Act as a resource for provision of information on mental health problems, care and treatment for older people.
  • Contribute to the development and delivery of education / training packages.
  • Maintain accurate, up to date clinical records.
  • Ensure records are overseen by a Registered Nurse.
  • Ensure confidentiality by storing and transporting records in accordance with the Trust’s policies and the NMC Code of Conduct.
