An exciting opportunity has arisen for A Registered Community Mental Health B6 Nurse to join our dedicated Memory Team at Monarch House in Scunthorpe. As a team are proud to of achieved Accreditation with the Memory Service National Accreditation Programme (MSNAP)
Our Memory Team at Monarch House comprises of 2 Consultant Psychiatrists, Nurse Consultant, Clinical Nurse Specialist, Registered Mental Health Nurses B6 & B5, Associate Nurse B4, Support Time Recovery Workers B3 as well as a Lead Occupational Therapist, Occupational Therapists & Occupational Therapy Technical Assistant.
This post is an exciting opportunity for a qualified registered mental health nurse who is looking to develop their knowledge and skills by working within a community based setting. You may be currently working within other areas of Older Adult or Adult Mental Health; however, this would give you a fantastic opportunity to join a very Specialised Experienced Older Adult Mental Health Team in a Community Setting.
Main duties of the job
Completing assessments, delivering non-complex diagnosis to patients.
Managing a caseload of vulnerable older adults with ongoing dementia symptoms.
Attending weekly Clinical Decision Discussion Meetings, Multidisciplinary meetings.
Organise and arrange Section 117 meetings.
Mentoring nursing students and allied health care professionals.
Weekly clinics & or home visits for ongoing reviews.
Monitor Memory/Antipsychotic Medication.
Work collaboratively with outside agencies.
Transferring patient care to the Primary Care Network Team.
Completing Incident Reporting Forms.
Completing Mortality Forms.
Attend Managerial and Clinical supervision every 6 weeks.

Qualifications
Essential criteria
- Registered Mental Health Nurse (RMHN)
- Evidence of continuous post registration training
- Mandatory training, including managing violence and aggression
Desirable criteria
- Evidence of post basic training in psychosocial interventions
- E.N.B. 998 or equivalent
- Non-Medical Prescribing (NMP)
Applicant requirements
You must have appropriate UK professional registration.
