Job overview

Mental Health Services in North Lincolnshire have benefited from over £3m worth of additional investment, which has enabled us to expand our Hospital Liaison Mental Health Team to operate to a Core24 Liaison Mental Health Service Model to Scunthorpe General Hospital as part of NHS England’s ‘Achieving Better Access to 24/7 Urgent and Emergency Mental Health Care’ Guidance.

The Hospital Liaison Team is commissioned to work in a predominantly nursing-led consultation model, comprised of a Nurse Consultant, Clinical Nurse Specialists, and Band 6 Nurses. with wider multi-disciplinary input from Consultant Psychiatrists and a Principal Psychologist.

We would welcome colleagues with an Older adult Nursing background, or younger adults  although the service covers across all ages ( 18+)

The Hospital Liaison Team provide assessment and support to service users referred into our team by Scunthorpe General Hospital. Scunthorpe General Hospital is currently in the process of being extensively refurbished and modernised, designed in consultation with our Hospital Liaison Team to ensure we have dedicated office and consultation space to carry out assessments within the Emergency Department. 

Main duties of the job

  • To have a wide base of clinical skills, knowledge and experience appropriate to the needs of individual adults and to the clinical setting in which they are required to be seen.
  • To work as a member of a Multi-Disciplinary Team reporting back to colleagues, both verbally and in writing and seeking advice, guidance and support where necessary.
  • To provide each patient with an agreed plan of care, containing review dates, in accordance with Community Mental Health Nurse and Care Programme Approach standards.
  • To actively seek to contribute to setting standards of care, to generate and implement quality initiatives.
  • To promote compliance with medication and other treatments prescribed, noting and reporting non-compliance and side effects as appropriate.
  • To develop and maintain healthy partnerships with all stakeholders including statutory, third sector and voluntary agencies.
  • To advise patients, carers and significant others of the available resources. Referring assessed need to those agencies best able to meet these in the community and facilitate use of these recourses.
  • To understand the requirements of the Mental Health Act 1983, Mental Capacity Act 2005, Care Act 2014, Care Programme Approach, Deprivation of Liberty safeguards as it applies to a psychiatric liaison service.
  • To participate in clinical and managerial supervision and Performance and Development Review.
