Job overview

An exciting opportunity has arisen to recruit an experienced, highly motivated and enthusiastic Physiotherapist within the Home First Service.

The Home First Service is a community based, multidisciplinary team (MDT) who provide health and social care for patients in Doncaster. Clinically you will work with patients in their own homes or in care home settings as an autonomous Physiotherapy practitioner. A key benefit of this post is that after 18-24 months in role we will support you to develop your career into Advancing Practice. With access to the required clinical supervision and study leave to support this development opportunity.

The Physiotherapist will provide high level clinical expertise, knowledge and skills in the assessment and treatment of patients in both the UCR service and the Virtual Wards. You will work alongside the wider MDT to optimise the rehabilitation and recovery of your patients, enabling them to remain at home and function to their best ability in their community. 

The candidate will be expected to work as an effective and responsible member of the team, supporting and developing others within the wider MDT and be open to new and innovative ways of working. 

Due to the nature of the post, provisionally selected applicants will be asked to apply for a disclosure from the Criminal Records Bureau.

Main duties of the job

 

          To assess patient understanding of assessment and treatment in accordance with the Consent Policy, and to assist members of the team to work within the legal framework for patients who lack capacity to consent to treatment.

          To provide specialist physiotherapy assessments over a range of conditions and complexities using advanced clinical reasoning to analyse clinical and non-clinical findings to form accurate physiotherapy diagnoses and prognoses.

          To provide health education, health promotion and prevention advice.

          To manage a clinical caseload, organising this effectively and efficiently with regard to clinical priorities and use of time with access to expert support as required, both for self and more junior staff, within MDT.

          To formulate and recommend to others the best course of physiotherapy intervention and/or treatment programmes to meet the agreed goals set with the individual patient/or carer, using evidence based practice and a wide range of treatment skills and options.

          To evaluate patient progress, review goal setting and alter treatment programmes as required.

Qualifications

Essential criteria
  • HCPC registered
  • Degree or Diploma in Physiotherapy
Desirable criteria
  • Relevant Master level post-graduate modules.
  • Member ofthe Chartered Society of Physiotherapy (CSP)

Experience

Essential criteria
  • Demonstrable appropriate post graduate experience (minimum 2 years post graduate experience as a physiotherapist in a broad range of areas including adequate relevant experience in the specific clinical specialty
  • Evidence of relevant CPD and a commitment to progress into advancing practice.
