Mental Health Practitioner - Rough Sleepers

Job overview

The Rough Sleepers Project- following this pilot project undertaken within Doncaster, we are able to offer this post as a permanent role after demonstrating positive impact for those identified people .

The NHS Long Term Plan acknowledges the importance of a focus on homelessness and issues related to supporting people with Complex Lives.

There is focus on health inequalities specifically relating to Homelessness and commitment to improve access to specialist homelessness NHS mental health support, integrated with existing outreach services.  In addition, it also refers a focus on severe mental health problems and commitment to a community-based offer including psychological therapies. 

This post will work as part of the Complex Lives Team with this post specifically will be hosted by RDaSH (allowing you to be based and aligned to the Complex Lives team but with the benefits of being part of the RDaSH Trust). The potholder will work closely with a peer support worker and mental health social worker coupled with full integration of the Complex Lives Team which includes team manager, social worker and navigators.

Your base will be in Doncaster with the Complex Lives team at the Civic Building in Doncaster.

Main duties of the job

This post is specifically to work with those people who are ‘rough sleepers’ in the borough of Doncaster.

The post holder will work as a member of a multi-agency and multi-disciplinary services working closely with the Complex Lives Team and community mental health teams, providing assessments of mental health and supporting access to mental health services as required. 

The post holder will carry out initial assessments and planning of group and/or individual therapy interventions in collaboration with adults who access services who are considered multiply disadvantaged, generally identified via the complex lives team, where concerns are raised in relation to mental health presentation.

To be responsible for the co-ordination of care needs, planning and implementation of evidence-based care to a defined group of service users in relation to mental health.

To provide professional advice and support to other agencies and members of the multidisciplinary team.

To act as a role model and resource for other members of the Complex Lives Team.
