Our team is undergoing significant expansion and are looking for highly motivated nurses to join us to provide specialist advice for adults referred with palliative care needs . This includes patients with malignant and non- malignant diagnosis.
The post holder will utilize skills of clinical expertise, leadership, education and management to ensure a seamless service for patients, those important to them and to colleagues. The requires working closely with the multidisciplinary team in hospital and the community to ensure a co-ordinated quality service.
Although based on the Weston site the post holder may be required to work across the Trust on occasions.

Provide specialist advice and support on complex pain and symptom management for patients with life limiting conditions and for those closest t them, for colleagues within and external to the Trust.
Be responsible for the assessment, planning and evaluating of specialist, individual management plans.
Working autonomously, demonstrate high levels of expertise and initiative within complex situations involving multiple teams.
Work closely with the patient and those closest to them to convey complex, sensitive information around prognosis and future care options.
Provide specialist education and training to the wider team who are providing direct core level palliative care to the person.
Lead and participate in the planning, development and evaluation of clinical services using advanced clinical skills and knowledge

Qualifications

- RGN and current NMC registration
- First degree or working towards
- 1st Degree in nursing or health related subject
- Relevant accredited post registration education at level 3
- Qualification in Teaching and Assessing
- Evidence of masters level academic study or willingness to work towards Masters (in relevant health related subject)

- Non-Medical Prescribing Qualification
Knowledge and Experience

- Thorough and up to date knowledge of nursing theory and best practice particularly within speciality or related area
- Relevant clinical experience
