Outreach and Support Worker

To undertake brief assessments and interventions with young people and their parents/carers and provide assertive outreach within the community.
To contribute to multi-disciplinary risk assessments.
To plan and review interventions with the child/young person and support network alongside the team in line with their Assessment, Safety, Care and Support Plan.
To work collaboratively with young people and their parents/carers and support network to further support the development of strengths as well as addressing risk/concern factors.
Respond sensitively to the needs of individuals with regards to their age, culture, race, gender, ethnicity, social class or disability.
Promote independence and increase self-confidence through developing appropriate self-help resources.
Support the team in developing practice resources.
Contribute to the protection of individuals from abuse and harm in line with local safeguarding policies and procedures.
Develop knowledge and practice relevant to the role through participating in training and supervision and accessing peer support.
Keep accurate records and complete timely reports as required

to hold an appropriate NVQ level 3 qualification in mental health, social work, criminal justice or youth work and/or be able to demonstrate an equivalent and relevant level of experience and knowledge alongside a commitment to ongoing professional development.

Experience of working with children and young people for whom services find challenging to engage as well as their parents/carers.
Relevant experience of working in any health, social care, education, criminal justice, or any related voluntary organisation setting.

The ability to form and sustain a working relationship with service users and parents/carers, demonstrating an understanding of when a more assertive approach might be needed.
An understanding of and commitment to anti-discriminatory practice.
Demonstrate an ability to deliver a range of specific approaches to assessment and therapeutic interventions with agreed outcomes within a prescribed framework relevant to this group of children and young people and their parents/carers/families.
Demonstrate well-developed listening skills and effective verbal communication skills, such as being able to engage appropriately with service users, carers, colleagues, statutory and voluntary agencies, both face to face and by telephone.
Demonstrate well-developed written communication skills, with the ability to produce letters, written reports, maintain accurate records and record required statistical information.
Able to demonstrate a general understanding of the issues relevant to children and young people who have engaged in problematic/harmful sexual behaviour and their parents/carers/families with a positive, non-judgemental approach

Background

Every three minutes someone in the UK develops dementia: a progressive and complex condition that can be devastating for the whole family. One in two of us will be affected by dementia in our lifetime – either by caring for someone with the condition, developing it ourselves, or both.

Dementia UK is the specialist dementia nursing charity that is there for the whole family. Our specialist nurses, known as Admiral Nurses, provide free, expert advice, support and understanding to help families care for their loved one. Every day, Admiral Nurses help families up and down the country to have the best life possible, for as long as possible.

Purpose of the job/about the role

Dementia UK has entered into an exciting three-year corporate partnership with a national partner. As a key component of this initiative, we will establish face-to-face Admiral Nurse clinics across the UK. This new team will bring Admiral Nursing to high street locations across England, Scotland, Wales, and Northern Ireland.

As part of the national Clinics Programme Team, you will provide in-person biopsychosocial support and advice, sharing your specialist dementia nursing expertise to help families manage the complexities of the condition. Your focus will be on supporting family carers, people affected by dementia, and professionals who require specialist dementia guidance in areas of the UK where Admiral Nurse services are currently unavailable, bringing a much-needed service directly to local communities. Throughout your work, you will consistently promote Admiral Nursing and a family-centred approach to dementia care.

You will receive full support with your continuous professional development from the Admiral Nurse Academy.

As this unique nursing role offers you the opportunity to travel across the country, and will include overnight stays, flexibility is essential. When not delivering face-to-face clinics, you will provide support on our Helpline, working from home.

Admiral Nurse – Delivered Services

Join our Delivered Services team as an Admiral Nurse and bring your specialist dementia nursing expertise to support family carers, people affected by dementia, and professionals who require specialist advice. You’ll be a key member of our team, providing biopsychosocial support through our National Dementia Helpline and Virtual Clinic appointments.

In addition to your core responsibilities, you’ll have the opportunity to lead and support other clinical projects across the charity, benefit from monthly clinical supervision, and participate in a range of learning opportunities to support your ongoing professional development.

As an Admiral Nurse, you’ll be responsible for providing nursing support and expertise, offering practical guidance to colleagues, the general public, those with dementia, and others in supporting family and relationship-centered approaches to dementia care. You’ll also lead and coordinate shifts as required, providing clinical supervision to others and assist with the development, evaluation, audit of the service, carer and supporters in different settings.

To be successful in this position, you’ll need to be an active Registered Nurse on the NMC register with a post-registration qualification or training in dementia care or a relevant subject. You will have significant experience of practicing as an Admiral Nurse or dementia specialist nurse and previously worked with people who have dementia and their family, carer and supporters in different settings.

Strong communication skills are essential, as is a proven track record of providing high standards of clinical leadership and specialist nursing support. You should be adept at providing clinical advice, clinical supervision, and advocating for family carers.
