You will be required to work as part of a busy community nursing team based around all areas of Telford who provide nursing care to patients who are housebound in the community. The post requires you to have a full UK driving licence and your own car. You must have excellent phlebotomy skills and experience in phlebotomy for this role as completing blood samples will be the main clinical tasks required.

Main duties of the job
- Providing patient care to patients within the community, often in patients own homes.
- Undertake delegated wound care and wound photography for patients with simple / routine care needs with appropriate training according to plan of care for the patient.
- Provide delegated simple continence support to patients such as catheter management education with appropriate training according to the plan of care for the patient.
- Work under the supervision of a community nurse escalating any patient risks or deviation to the plan of care when required.
- To organise own phlebotomy workload in the most time effective way whilst meeting the priority time scale requested by the referrer.
- Undertake community phlebotomy within the priority timescale requested by the referrer and to patients who are unable to access a phlebotomy clinic, escalating any inappropriate referrals to the service.
- Obtain specimen samples as directed and ensure all specimens are transported to the laboratory or point of sample collection within Trust policy and procedures.
- Assist in the holistic care of patients by escalating any concerns with the patients wider physical, social, psychological care needs / activities of daily living.

Knowledge and Skills
Essential criteria
- Knowledge of the role of the health care assistant role in context, including phlebotomy.
- Good interpersonal skills (written and oral).
- Knowledge of situations and circumstances that can impact on patient safety and demonstrate understanding of how the healthcare support worker contributes to maintaining patient safety.
- The ability to communicate effectively with patients and staff both written and orally and demonstrate a level of English language and grammar commensurate with writing in service user case notes.
- Experience of using IT in clinical practice.

- Ability to meet the travel needs of the post, within community nursing teams, the post holder will be required to travel to patients homes.
Qualifications
Essential criteria
- GCSE Level C+ in English and Maths or Level 2 functional skills in English and Maths or willingness to work towards this.
- Level 2 in health and social care (or equivalent) or willingness to work towards this.
- Care Certificate or willingness to work towards this.
Experience
Essential criteria
- Awareness of accountability issues e.g. confidentiality.
- Awareness of the role of the Healthcare Support Worker.
