An exciting opportunity has arisen for a Nurse Associate to join the Central Multidisciplinary Team in the provision of Urgent and Planned Care to patients living in the community.  The multidisciplinary teams provide nursing and therapy services offering an alternative to hospital admission, supporting timely discharge. We enable our patients to realise their potential independence in their most appropriate place of care.

As a Nurse Associate, you will be supported to develop a broad range of clinical skills to enable you to provide evidence based care. You will receive additional training and clinical support specifically in regard to Continence Care & Assessment and will work with the Continence Nurse to enable timely care to a delegated group of patients. You will need to be qualified to AN level, be registered with the NMC and be able to demonstrate current clinical experience. We are committed to clinical support, CPD & training.

You will need to be able to meet the travel requirements of the post.

  • Provide compassionate, safe and effective care and support to people in a range of care settings.
  • Ensure the privacy, dignity and safety of individuals is maintained at all times.
  • Monitor the condition and health needs of people within their care on a continual basis in partnership with the members of the MDT team families, and carers.
  • To complete nursing risk assessments using recognise risk assessment tools.
  • Contribute to the planning of care delivery in association with the Registered Nurse .
  • Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment and action.
  • Once passed as competent to administer medicines as directed.
  • Demonstrate the ability to recognise changing priorities seeking advice and guidance from the Registered Nurse or other registered care professionals as appropriate.
  • Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care.
  • To oversee the care delivered to their designated group of patients by Health Care Assistants. Trainee Nursing Associates and Nursing Students.
  • Recognise and report any situations, behaviours or errors that could result in poor care outcomes.

Qualifications

  • Registered Nursing Associate on the NMC register
  • Educated to level 5 Diploma of Higher Education and Foundation degree in nursing associate practice / Nurse Associate Apprenticeship Certificate.

Experience

  • Ability to work effectively as a team player under appropriate supervision, and as part of a multi-disciplinary team.
  • Understanding of the scope of the role of the Nursing Associate in context of the team and the organisation, and how the role contributes to service development.
  • Demonstrable experience of working as an Nurse Associate within the NHS or other care setting.

Skills
