Job overview

As a Caseload Holder you will manage a designated group of patients to ensure that they receive timely and evidence based care. You will have a skill mixed team to support the delivery of this care. You will need to be qualified to RN level, be registered with the NMC and be able to demonstrate current clinical experience in a community setting. We are committed to clinical support, CPD & training.

There is a need for excellent communication skills and a dedication to multi agency working; you will need to work flexibly within a busy, demanding environment with the ability to develop new skills and working patterns.

Main duties of the job

  • To adhere to the NMC Guidelines for Professional Practice.
  • To meet the NMC Standards for Records and Record Keeping, whilst 
    exercising confidentiality and discretion.
  • To maintain professional competencies as required under Continuous 
    Professional Development.
  • To attend mandatory training as identified by the Shropshire Community Health NHS Trust and be responsible for team members attendance to ensure that quality standards are maintained.
  • To develop and maintain effective communication networks with other health professionals, private, statutory and voluntary agencies with regard to patient care.
  • To have knowledge of, and adhere to Shropshire Community Health NHS Trust Policies and Clinical Guidelines.
  • To participate in research and initiate and participate in clinical audit of services to improve practice and contribute to the development of clinical procedures and guidelines.

Experience

  • Wide range of nursing experience within a community setting.
  • Evidence of personal and professional development.
  • Mentor qualification and experience of undertaking clinical assessment.
  • Clinical Supervision – supervisor level.

Knowledge and Skills

Essential criteria
  • IT Skills or willingness to undertake ECDL training.
  • Broad knowledge of current community issues and research within nursing.
  • Leadership skills.
  • Proactive approach & innovative.
