This is an exciting time to join Oxford Health NHS Foundation Trust pharmacy team, as we move to a new electronic prescribing and medicines administration system.

Main duties of the job

 We are looking for a passionate and driven Clinical Pharmacist to help develop an outstanding pharmacy service for our Oxfordshire Community Hospitals.  We currently have wards in Abingdon, Witney, Didcot, Wallingford, Bicester and Oxford.

We are committed to developing a patient-centred approach for all our patients across our community hospitals, and to be an integral part of the multidisciplinary team of healthcare professionals. 

The role involves;

• Regular clinical visits to inpatient wards
• Structured/medication reviews
• Clinical screening of medication charts
• Clinical audit
• Developing pharmaceutical care plans
• Providing advice and training to other healthcare professionals
• Discharge medicines planning with patients/family
• Attendance at ward rounds and multidisciplinary team meetings

The role will include some cover in the pharmacy dispensary, participating in weekend working and the on-call pharmacy service on a rota basis, as well as working on our Community Hospital wards.

Detailed job description and main responsibilities

Applicants must be qualified pharmacists registered with the GPhC and possess good communication and interpersonal skills. You would need to be able to work independently as well as in a team in a busy ward environment.
Applicants will also need to be able to commute between sites.

Qualifications

Essential criteria
  • Masters degree (or equivalent) in Pharmacy
  • GPhC registration
Desirable criteria
  • Postgraduate Clinical Pharmacy Diploma

Experience

Essential criteria
  • Significant post-qualification experience in pharmacy
Desirable criteria
  • Experience in hospital pharmacy

Knowledge & Experience

Essential criteria
  • Competence in the use of Microsoft Office (i.e.spreadsheets word processing, presentation software
  • Ability to use a Pharmacy patient record and supply system
  • Ability to convey complex, sensitive information to other Healthcare professionals and patients.
Desirable criteria
  • Ability to manipulate and analyse electronic data.
