Trosolwg o'r swydd

Oxford Health Foundation Trust Forensic Service is developing and delivering a new integrated model of mental health care for men in prisons across the region. We are looking for forward thinking, innovative, creative, compassionate and thoughtful senior mental health nurses who will be able to offer a wide range of interventions in HMP Grendon near Aylesbury, which is a Category B prison, operating as a therapeutic community for approximately 240 men. 

You will be managing a caseload of patients in a person-centred way to plan, deliver and evaluate care, whilst being supported by a well-resourced multidisciplinary team. You will be able to develop your own practice, and support others to develop theirs, in partnership with primary health care providers and prison staff.

 

Prif ddyletswyddau'r swydd

To provide mental health interventions to service users within the prison establishment.

To be responsible for managing a caseload, including the assessment, planning, delivery of interventions and review of care of patients located within the prison with a mental illness, within the framework of the multi-disciplinary team and Care Programme Approach.

To work as an integral member of the multi-disciplinary, multi-agency team to provide advice and support to primary health care based within the prison, and to liaise with and provide advice and support to, statutory and non-statutory agencies.

To support the team manager in delivering identified objectives in terms of patient care, staff and service development.
