As a Physical Health Care Assistant in the SMI team at Oxford Health NHS Foundation Trust, you will play a crucial role in supporting patients with Severe Mental Illnesses (SMI) by managing their physical health needs. Your main duties will include:

·        Conducting Assessments: Perform physical health assessments and interventions, including monitoring vital signs like blood pressure and pulse, and conducting blood tests.

·        Patient Care: Provide care in various settings, including community bases and patient homes, responding to immediate patient needs and reporting changes to healthcare professionals.

·        Documentation and Reporting: Accurately record and report all care given, ensuring that information is shared with appropriate healthcare professionals and maintaining patient confidentiality.

·        IT Utilization: Use IT and data systems effectively for managing patient records and communications.

·        Patient Education: Educate patients and their carers on health and well-being, local resources, and treatment goals.

·        Collaboration and Support: Work closely with other team members and contribute to patient care plans, promoting a holistic approach.

·        Flexibility and Adaptation: Adapt to varied settings and work flexibly according to patient needs and service requirements.

·        Training and Development: Engage in ongoing training and clinical supervision to maintain and enhance your skills and knowledge.

·        Service Promotion: Represent and promote the service across different sectors including secondary care, social services, and community resources.

  • The post holder will be expected to attend and fully complete a nationally agreed induction programme and undertake an on-going training to achieve a minimum of NVQ2 / VRQ
  • Good level of secondary education with demonstrable literacy and numeracy skills

  • NVQ level 2 or 3 or other relevant qualifications

Further Training or Job Related Aptitude and Skills

Experience

  • Previous experience working in a care or rehabilitation setting.
  • Experience of using Office computer programs including Excel, Word and Outlook
