The Single Point of Access (SPA) team is seeking an experienced nurse or HCP to join our team as a Clinical Lead in the South Hub based at Abingdon Community Hospital. SPA is a service that manages the inbound referrals for multiple community teams and is both public facing and for health professionals to use. Informal visits and contact with the recruiting manager are encouraged to find out more about this unique service.

You will work with the Operational Manager, Modern Matron and peer Clinical Lead in leading the community SPA team to maintain business as usual as well as leading on aspects of the transformation works linked to national drivers.

This post is 37.5hrs per week and you will be expected to work predominantly week days, with the occasional weekend or bank holiday shifts based on service need. This role will require you to travel between rural community bases across Oxfordshire including the Banbury office at the Fiennes Centre. You must therefore be comfortable with driving for work purposes and able to travel freely within the hours of the role. Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required).

This role includes several core duties, some of the essential which you will need to demonstrate knowledge and experience in are line management of staff, recruitment, problem solving skills, and quality improvement/ service improvement. You will have overall responsibility for providing guidance, support and mentorship to clinical and non-clinical staff, overseeing the systems and processes in place to ensure the safe and efficient referral management and problem-solving issues as they arise throughout the day.

You will be expected to work in partnership with acute, primary and community HCP’s with system navigation and managing the inbound community referrals for Oxford Health.  You will work in synergy with the team providing guidance, support and mentorship to staff. You will be core to the continuous service development and transformation including the implementation of local initiative and wider directorate level transformation workstreams.

The successful applicant will receive a comprehensive Induction to the service including the support of you line manager, senior clinical leadership and peer clinical lead.

Managerial

·        To lead the operational service delivery within the community working closely with acute sector, and Social Service colleagues adapting working style, flexing capacity, and implementing escalation measures to meet immediate and long-term pressures.

·        To lead the service development within the department working closely with community services, acute sector, and Social Service colleagues to ensure a strategic and operational fit both in the short and long term.

·        To be accountable for all team resources including the budget.

·        To deputise for the SPA Operational Manager when required.

·        To lead and be accountable for the implementation of performance indicators within the team

·        To lead and be accountable for the implementation of departmental audit seeking external support as appropriate.

·        To lead the development and implementation of service specific policies, procedures, and protocols.

·        To be an expert resource to partner professionals, services, and organisations.

Clinical

·        To maintain advanced knowledge and understanding of health and social services, including their access criteria and availability.  This includes community hospitals and a range of specialist services e.g. continuing or palliative care.

·        To actively work with the community service leads to facilitate and forward plan transfers of patients to community beds and other community services.

·        At times of escalation to take emergency level decision making to facilitate the transfer of patients to optimise available capacity across the whole system using escalation measures as appropriate.

·        To contribute specialised knowledge of community and intermediate care services to the daily Whole system conference to ensure effective flow of patients within the whole system.

·        To maintain accurate patient and service records sharing information as appropriate and in line with Caldicott principles.

·        To provide a daily report of patient flows to relevant managers, senior managers and chief executives in both health and social care.

·        To lead service specific audit and participate in wider research / audit initiatives to inform future service development.

·        To lead the development of and maintain close working links with all partner services and agencies.

·        To have an in depth understanding of local and national policies in order to contribute to the development of practice.

·        To lead the development of local operational policy to support best practice and the delivery of quality service.

·        To work in partnership with service manager and relevant colleagues to ensure adequate service provision as agreed and to meet the local service standard of assessing all patients within 24 hours of referral.

Professional

·        To work within all Trust policies and procedures, and your professional  code of conduct.

·        To lead the development of the individuals within the nursing team including individual performance reviews and the development and implementation of personal development plans.

·        To participate in clinical supervision as agreed.

·       To undertake on-going professional development in line with the needs of the post and requirement of professional bodies to maintain clinical knowledge and experience.
