Job overview

The Community Neurological Rehab Service have an exciting opportunity for a dynamic and skilled clinician to join our well established team as a Specialist Neurological Nurse. 

As a registered nurse you will work as an autonomous practitioner, working collaboratively within an integrated multi-disciplinary team, supported by the Clinical Team Leader. The successful candidate will be motivated, forward thinking, enthusiastic and committed to becoming involved in all aspects of the patient pathway. 

You will assist in managing a complex caseload and provide expert assessment and long term condition management  for patients with long term neurological conditions. Input will be provided across the disease trajectory from education at onset through to mild and moderate complex disability. Care is delivered in patients own homes/usual place of residence for the adult population of Mansfield, Ashfield, Newark & Sherwood.

You will play a key part in the development of long term condition management within community services and ensuring commissioned targets & Key Performance Indicators are met.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. 

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies. 

Main duties of the job

To provide a defined level of specialist assessment, condition management and access to interventions for patients who present with Long Term Neurological Conditions.

To provide a realistic, timely and occasionally intensive support to patients within their own community setting. 

To demonstrate active clinical leadership skills within the team, offering clinical support and advice and providing education and supervision to junior staff members and other professionals, if required.

Detailed job description and main responsibilities

Are you a registered nurse with experience in neurology looking for a new challenge? If so we would love to hear from you!

We are looking to recruit an enthusiastic, passionate and dynamic individual to work within our neurological nursing team to support and manage patients living with long term neurological conditions. You will need to possess community nursing knowledge and have some experience of neurological care.

The team are based at Warsop Primary Care Centre and cover the geographical location of Mansfield, Ashfield, Newark & Sherwood. The service operates Monday-Friday 8.00 -5.00pm  (excluding bank holidays). 

The Community Neuro Team consists of Specialist Nurses, Specialist Physiotherapists and Occupational Therapists, Speech and Language Therapists, Clinical Psychologists, Assistant Practitioners and Community Rehab Assistants.

As a specialist neurological nurse you will:

Deliver high quality, safe, effective and efficient service to meet the needs of the defined patient group through support, kindness and respect.

You must be an NMC Registered Nurse with Neurological experience and have evidence of continuing professional development, knowledge of working with people with complex physical health problems, to inform and support the clinical care pathways; preferably within a community setting although clinical skills and experience are transferable.

You will need to be highly motivated, have positive effective interpersonal and management skills and a commitment to promote recovery and rehabilitation orientated practice with the aim of preventing unnecessary and reduce hospital admissions in a variety of ways.

You will work closely with other members of the wider MDT, including our acute colleagues, GPs and other health professionals in order to maximise patients quality of life and encouraging self management. 

Due to the nature of the post a full UK driving licence and vehicle for business use is required in order to travel independently across the geographical area; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
