Community Rehabilitation Assistant

Bassetlaw Urgent community response ( UCR) team are looking to recruit an enthusiastic and proactive individual to join our team in a Community Rehabilitation Assistant role.
Our service offers hospital, at home and intensive rehabilitation services to people requiring sub-acute care and physical rehabilitation to prevent hospital admission. Our team consists of Clinical Assessors, Physiotherapists, Occupational Therapists, Assistant Practitioners and Community Rehabilitation Assistant providing nursing and rehabilitation support for adults to enable them to improve their function, maintain their independence and achieve their goals.

The duties of this post include carrying out delegated aspects of the nursing and therapy treatment programme, supervising and progressing patients goals, working independently in the patients own homes, and working as part of the multi-disciplinary team. We are looking for someone with good communication and organisational skills, who has the ability and flexibility to work as part of the multi-disciplinary team. The post involves weekend working & shift work - the current hours of the service are 7am-9 pm. A full programme of training would be available for the successful applicant.
A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
