Job overview

As a result of the extra investment that is coming into Mental health services as part of the NHS Long term Plan, we are excited to be able to expand and transform what we offer to this service user group and their families. You will work as part of a community rehabilitation team as part of the wider rehab pathway offering interventions that are in line with National guidance.

The post holder is identified as having specialist knowledge in the mental health needs of adults and their families with specific focus and remit to deliver significant improvements for patients that have long term mental health conditions and rehabilitation needs.

Main duties of the job

To provide the Community Rehabilitation Team (CRT) with the defined level of expertise as a registered Mental Health Nurse. 

This includes providing a specific expertise in assessment and care planning for service users within the CRT service.  Advocate rights of individuals to live independently, with their families, with careers or with placements. Continue to work with people with mental health needs by working with the service users in a holistic and person-centred approach.

To provide a lead role as a Mental Health Nurse when working with fellow colleagues within the community rehabilitation team.  Assist the team in assessments and formulations, care/management/crisis and placements, this includes inpatient units within the trust and wider placements within the local authority.  Taking a particular lead in medication support/management or those service users under national legislation of the Mental Health Act. aswell as and placement plans to enable to CRT to assist service users transition from in-patient to discharge back into the community.

To maintain a caseload of service users within the CRT, attend group formulation/planning meetings and support fellow colleagues with formulation. Undertaking activities with service users in the community.

To provide realistic, timely and occasionally intensive Nurse led interventions to patients within a community setting.

The role will be working with service users with a serious or complex mental health illness.

Detailed job description and main responsibilities

To develop and maintain service relationships with internal services and external organisations including the local authority and external NHS or private healthcare providers.

To maintain a caseload of service users within the CRT, attend group formulation/planning meetings and support fellow colleagues with formulation. Undertaking activities with service users in the community.

To provide realistic, timely and occasionally intensive Nurse led interventions to patients within a community setting.

The role will be working with serviced users with a serious or complex mental health illness.

This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust.  This is payable in 3 instalments over a 2 year period.  This payment is only eligible to new starters who do not already hold a contract of employment with the Trust and have not been an employee of the Trust in the previous 3 years.  This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays

Person specification

Qualifications

Essential criteria
  • Mental health Nursing Qualification to degree level or equivalent plus significant further specialist knowledge.
  • Maintains current registration and adheres to professional standards.

Experience

Essential criteria
  • Working with people experiences serious and complex mental difficulties.
  • Management of caseload of service users in the community, inpatient and placements.
  • Assessment, and relevant interventions and review of care packages for service users referred to the team.
  • Experience of working in MDT
Desirable criteria
  • Demonstrate an understanding of physiological healthcare and have the ability to take appropriate assessments and action i.e. signposting for diabetes, epilepsy
  • Experience in rehabilitation services.

Knowledge and Skills

Essential criteria
  • Knowledge of current relevant local and National Legislation including the Capacity Act.
  • Awareness and knowledge of trauma informed services and psychological interventions for those experiencing SMI
  • Appreciation of the importance of the working with families and carers.
  • Working knowledge of CPA, mental health act, mental capacity act, deprivation of liberty, safeguarding etc.
  • Demonstrate an ability to carry out a comprehensive assessment of specialist health, well-being and social care needs.
  • Ability to form and maintain therapeutic relationships.
  • Risk assessment and safety planning.
Desirable criteria
  • Knowledge/accreditation of assessing students in clinical practice.
