Advanced Clinical Practitioner Health Care of the Older Person

Job overview

We have an opportunity for a highly motivated, knowledgeable and enthusiastic qualified Advanced Clinical Practitioner (ACP) to join our Health Care of the Older Persons (HCOP) team.  Full time or part-time applicants are welcome to apply. 

The team consists of nine qualified ACPs and two trainee ACP’s, with a physiotherapy and nursing background. We support the emergency pathways (Medical Admissions/ soon to be new Geriatric Assessment Unit) however our main role is to support the seven acute HCOP wards across both sites on a rotational basis with desirable fixed working pattern.

Being one of the largest specialities within NUH, HCOP is a dynamic and ever changing environment with opportunities for innovation and expansion of our existing services to frail older adults both within the hospital and community.

This is an opportunity for any qualified  ACP who have an interest in geriatrics/HCOP, if you are an ACP in another speciality who is looking for a change and a new challenge we are able to support the transition from another speciality by offering supervision/supernumerary period to build on specific geriatric competencies/skills. Full time and part time applicants are welcome to apply. 

Main duties of the job

  •  Monday- Friday working 08:00 – 17:00, with some working weekend working (approximately every 6-8 weeks).
  •  Contribute to the medical rota working within the MDT
  •  Primarily based at QMC with an expectation to rotate to City Hospital as part of the team’s rotation ( 4 monthly rotations)
  •  Working on the Medical Admission/Geriatric Assessment Unit
  •  Opportunity to rotate/work on the medical mental health unit with complex mental health patients
  •         Implicating comprehensive geriatric assessment (CGA).
  •         Independent clinical assessment, diagnosis and treatment of patients with undiagnosed, undifferentiated conditions
  •         Emphasis on medication reviews and appropriate prescribing/de-prescribing
  •         Referring/liaising with other specialities
  •         Requesting appropriate investigations (inpatient and outpatient)
  •         Delivering high quality evidenced based patient centred care

  •        Protected continuing professional development (CPD) time.
  •         Audit opportunities (regional and national)
  •         Teaching opportunities (internal and external) – the team currently leads on the Nottingham Trent University frailty module
  •         Opportunity to attend HCOP specific junior doctor/medical teachings

  •         Opportunities to attend specialist HCOP clinics/services (eg. Orthogeriatrics, HCOP clinics (falls, bone health, Parkinson’s/Movement disorder)

Detailed job description and main responsibilities

Clinical Assessment:

 

  •   -      Conduct comprehensive assessments of frail and elderly patients, including physical, cognitive, and psychosocial evaluations.
  •         Formulate accurate diagnoses based on assessment findings and collaborate with the healthcare team to develop individualized care plans.
  •         Maintain comprehensive and accurate medical records and documentation that is consistent with legislation, policies and procedures

Management and treatment:

 

  •          Initiate and manage appropriate interventions for frailty-related conditions, chronic diseases, and geriatric syndromes.
  •          Prescribe medications, order diagnostic tests, and coordinate with other healthcare professionals to optimize patient outcomes.
  •          Undertake advanced (invasive and non-invasive) procedures pertaining to patient need, in accordance with appropriate training and clinical supervision
  •          Effectively manage acute life threatening conditions in accordance with current ALS

Holistic care:

 

  •         Implement a patient-centred approach, considering the unique needs and preferences of older adults, with a focus on maintaining functional independence and quality of life.
  •          Provide education and support to patients and their families regarding frailty management, geriatric care, and preventive measures.
  •         Communicating/leading on difficult conversations with patients, families/carers with regards to prognosis and end of life discussions

Multidisciplinary collaboration

 

  •          Work collaboratively with physicians, nurses, therapists, social workers, and other healthcare professionals to ensure coordinated and integrated care.
  •          Participate in regular case conferences and contribute to the development of care pathways for frail and elderly patients.

Quality improvements

 

  •          Engage in continuous quality improvement activities, including audits and research projects, to enhance the standard of care for frail and elderly populations.
  •          Stay abreast of current research and evidence-based practices in geriatrics and frailty management.

Leadership and mentorship:

 

  •          Provide leadership in the development and implementation of best practices in the care of frail elderly patients.
  •          Act as a mentor to junior healthcare staff, sharing expertise and promoting professional development

Qualifications

Essential criteria
  • Msc in Advanced Clinical Practice
  • Registered Nurse or Allied Health Professional registered with a professional body
  • Non Medical Prescriber registered with NMC or other professional body
  • ALS Provider
  • Evidence of post registration training and/or qualifications
  • Evidence of teaching, supervision and assessment skills
Desirable criteria
  • ALS Instructor
  • Mentor/assessor qualification

Experience

Essential criteria
  • Completed a portfolio of supervised practice in Advanced Clinical Practice
  • Evidence of continued professional development following completion of Msc in Advanced Clinical Practice
