Lead Nurse for Capacity and Patient Flow

Working with a vision of “delivering excellence in all everything we do”, the post holder will lead the site operations team across NUH. This role will ensure the site team are supported in placing the right patients, in the right place, at the right time and in doing so support the delivery of government targets for ‘patient access standards’.
Specifically, the post holder will be responsible for providing senior professional, operational and strategic clinical leadership to the site operations team, leading and developing this team to support 24/7 patient placement across the organisation.
The post holder will proactively support patient flow from the point of patient admission through to discharge, liaising with relevant clinical teams and divisions as necessary to minimise delays and problem solve flow issues. This role will operate as Silver on Call during the day and will work closely with Silver on Call colleagues in line with escalation levels.
The post holder will develop excellent working relationships with clinical teams across the Trust and will deputise for the Head of Emergency Performance/Deputy Chief Operating Officer as required.
In undertaking the duties for this post the post holder must comply with the Nursing and Midwifery Council’s professional standards ‘The Code’.
The post holder will be based at both the City Hospital and the Queen’s Medical Centre. There is an expectation to participate in a Trust on-call rota as part of this role.

The post holder is responsible for ensuring issues raised at operational meetings are completed/resolved and action outcomes feedback or appropriately escalate.

KEY JOB RESPONSIBILITIES

Specifically, the post holder will:
1.1 Ensure staff working as part of the site operations team are clear about what is expected of them to ensure success and are working together effectively to achieve the Trust’s vision and delivering excellence in patient care.
1.2 Lead the communication process with other providers, establishing effective working relationships which are built on mutual respect and trust and can offer critical feedback associated with the discharge process.
1.3 Ensure all members of the on call teams receive an up to date status report regularly.
1.4 Work with Divisional teams to deliver escalation actions at times of pressure.
1.5 Remove constraints to patient pathways and ensure escalation of blockages to avoid breaches of the standards in emergency care, elective pathways and cancer targets, as well as preventing non-clinical cancellations of elective activity.
1.6 Lead the development and implementation of clinical policies and protocols.
1.7 Provide quality leadership to the Duty Matron team and to bronze on call teams to manage patient flow successfully across both sites.
1.8 Work closely with the IDT team and ensure supported discharges are timely, appropriate and managed effectively with partner organisations.
1.9 Champion the excellence agenda by demonstrating personal commitment to continuous improvement.


2. Service Delivery
2.1 Chair and lead regular escalation meetings during the day.
2.2 Undertake teleconferencing as required under escalation with partners from the wider health economy to escalate/discuss issues which may affect demand and capacity, developing an action plan accordingly to deliver a system response.
2.3 Produce daily reports for internal and external providers (e.g. NHS and A&E delivery board) detailing performance and contributing factors/mitigating actions.
2.4 Work closely with all clinical divisions on a daily and planning basis to achieve key performance measures, namely the standards in emergency care.
2.5 Assimilate and analyse large volumes of data regularly to produce meaningful information to inform regular site meetings throughout the day.
2.6 Support the Head of Emergency Performance and Deputy Chief Operating Officer in the interpretation of national guidance and translating it into service delivery plans to align to national direction and local priorities.
2.7 Develop and sustain relations, partnerships and networks to influence and improve health outcomes and healthcare delivery locally.
2.8 To be responsible for the oversight and management of the hospital site 24-7, including taking the lead of a critical and/or major incident until the on call manager arrives on site.
2.9 Manage nurse/medical related staffing issues and monitor staffing levels across all inpatient areas out of hours.
2.10 Investigate and respond to formal complaints regarding site operational matters in conjunction with relevant staff, within the allotted time period, implement changes to care delivery as required.
2.11 Work with the CCG and community providers to ensure delivery plans support transfer of care of NUH patients in a timely manner.
2.12 Work closely with Divisional General Managers, Deputy Divisional General Managers and Service General Managers to support services in other parts of the Trust using matrix management to ensure delivery plans are compatible and maximise the opportunities for more efficient ways of working.
2.13 Challenge existing practices, ensure that progressive solutions, which take into account models of best practice and best use of resources are incorporated into service plans to ensure continuous improvement of the service (and therefore patient care).
2.14 Write the rota for the site operations team ensuring there is 24-7 Duty Matron cover on both Queens and City Campuses.
2.15 Support Staff with re-design projects and action plans demonstrating a high level of commitment to meeting, if not exceeding, all expectations.
2.16 Budget holder for site operations team in which there is to be a constant strive for value for money and greater efficiency and to ensure recurrent financial balance year on year.


3. Performance management
3.1 Ensure the site operations team has clearly defined performance objectives supported by systems and processes which will deliver continuous improvement.
3.2 Hold Divisions to account for SAFER targets and any delays contributing to not achieving the standards in emergency care.
3.3 Achieve the national performance standards for emergency care.
3.4 Work with the Head of Emergency Performance/Deputy Chief Operating Officer and Deputy Chief Nurse to regularly review the effectiveness of patient pathways and to identify and rectify and blockages to performance.
3.5 Recruit the team to sustainability.
3.6 Enhance the performance and capability of others through formal and informal interactions.
3.7 Motivate staff to higher levels of performance by increasing their commitment to the Trusts’ objectives, clarifying the visions and developing effective team working.
3.8 Line-manage the Duty Matron Team (to include sickness, appraisals and mandatory training) and performance-manage Duty Matrons, if required.
3.9 Ensure the site operations team fully utilise the systems in place, not reproducing data, but
turning it into meaningful information.


4. Governance and Risk
4.1 Build and promote systems for clinical quality and safety improvement.
4.2 In conjunction with the Deputy Chief Operating Officer, lead the Governance and Risk agenda on behalf of the site operations team.
4.3 Develop and implement effective systems for recording and monitoring governance and risk information and provide reports to the appropriate forum.
4.4 Promote a culture where governance and risk management are seen to be everyone’s responsibility and ensure all staff are facilitated to report incidents promptly and receive timely feedback.
4.5 Ensure facilities and communications are established to involve the relevant external providers to provide feedback where necessary using tele/video conferencing facilities.


5. Education and teaching
5.1 Ensure all staff in the site operations team receives appropriate training and ongoing development to enable them to competently and safely fulfil their roles.
5.2 Ensure training is accessible to all clinical and non-clinical staff, in a variety of formats, recognising the diverse needs of the workforce.
5.3 Promote a culture of lifelong learning.
5.4 Provide constructive feedback, support and resources to enable the team (and wider teams) to develop and increase skills and capabilities.
5.5 Ensure that appropriate mechanisms are in place to achieve the robust induction and workplace orientation of all staff new to the service area.
5.6 Be responsible for the training and development of silver on call colleagues and ensure a rolling training programme is in place.


6. Communication
6.1 Provide and receive highly complex, highly sensitive and contentious information. Interpret and analyse information and be able to present this to a wide range of internal and external stakeholders in formal settings.
6.2 Manage potentially aggressive and/or antagonistic and challenging situations with staff and stakeholders to ensure successful outcomes.
6.3 Employ effective communication, negotiation and influencing skills to enable effective change management with all stakeholders at all levels (including senior management) who may hold differing and contentious views.
6.4 Ensure optimum engagement with all stakeholders, securing appropriate buy in, support and understanding in relation to safe patient placement and the management of the Trust’s bed capacity.
6.5 Be the lead as the patient placement expert, integrating systems and managing effective working relationships with the appropriate stakeholders.
