Supporting Patient Excellence:

 To lead the development and implementation of the dedicated nurse led prostate pathway service in line with national directives.

 To set up nurse led telephone clinic, triaging new patient referrals, ensuring all the relevant information is provided, liaising with GPs to ensure the template is completed.

 To ensure all the appropriate investigations are completed prior to the first clinic appointment.

 To undertake a clinical assessment of patients with suspected prostate cancer, including physical examination, assessment and investigation of symptoms and interpretation of results.

 To refer patients with identified holistic needs to the Macmillan Information & Support Service

 To order investigations including MSU, PSA, mp MRI, bone scan and CT as clinically indicated 

 To attend outpatient clinics, working collaboratively with the surgeon to provide written and verbal information for patients to support informed decision making.

 To counsel patients prior to invasive investigation procedures; including biopsy of the prostate providing written information to meet the individual needs of patients/carers.

 To act as a point of contact for patients during the diagnostic phase, referring onto the Macmillan Uro-oncology Team once a diagnosis of prostate cancer has been confirmed.

 To refer to other specialities following assessment and diagnostic such as incidental findings of other cancers.

 To develop written information for patients about the nurse led service, providing contact details for advice and support.

 To ensure appropriate treatments for patients are prescribed and signed for by the medical staff who require treatment following assessment and diagnostics, including hormone treatment, Benign Prostatic Hyperplasia (BPH), LUTS, UTI management. Non-medical prescribing is a desirable skill not essential for the post.

 Professional Responsibilities:

 To adhere to the NMC Code of Professional Conduct and other national regulations

 To undertake training to develop confidence and competence in performing transperineal l biopsies of the prostate, maintaining a portfolio of evidence to demonstrate extended practice.

 To complete and maintain IRMER competence to request radiological investigation, including CT, MRI and Bone Scans in line with Trust policy.

 To work autonomously as a nurse specialist, recognising own limitations and when to seek advice from the medical team.

 To act as a specialist resource for GP’s and other healthcare professionals relating to prostate cancer, providing expert nursing advice and support 

 To promote collaborative working relationships and effective communication with the members of the Uro-oncology multidisciplinary team and with Primary Care

 To act as a role model demonstrating expertise related to prostate cancer and in undertaking transperineal biopsies of the prostate.

 To have expert knowledge of the prostate pathway, working to agreed protocols.

 To demonstrate specialist knowledge in planning, implementing, and evaluating nursing care, documenting the outcomes of consultations in the hospital notes and on the Somerset Cancer Database.

 Demonstrate an ability to work across organisational and professional boundaries.

 To identity own personal development needs to work at an advanced level and take appropriate action to ensure that these are met

 To maintain a personal professional profile, demonstrating clear evidence for nursing revalidation

 Quality Assurance and Monitoring:

 To participate in the self-assessment against the national standards related to prostate cancer, taking part in the NHS England Quality Surveillance Program 

 To ensure the nurse led service is developed in line with national directives related to prostate cancer.

 To collect activity data to demonstrate the effectiveness of the specialist nursing service.

 To actively seek feedback from patients/carers to inform service developments.

 Policy and Service Development Responsibilities:

 To lead the development of policies and clinical guidelines to develop and implement the specialist nursing service in line with national directives. 

 To influence decisions regarding service delivery through participation in local, regional and national meetings

 To work in collaboration with the Cancer Clinical Director to provide education and training to GP’s referring into the nurse led service.

 To evaluate the effectiveness of the nurse led service, implementing change to enhance the patient/carer experience.

 To identify areas of practice that may be expanded to promote optimal care and the best outcomes for patients.

 Research and Development Responsibilities:

 To initiate and be actively involved in nursing research/audit.

 To promote best practice ensuring care is evidence based
