Kettering General Hospital NHS Foundation Trust is seeking a motivated and dynamic clinical pharmacist with an interest in frailty to join our established, friendly Frailty Team. Based in a busy emergency department, the Frailty Team aid frail and older adults in retaining their independence and, crucially, managing their medications.
The post holder will be responsible for providing a dedicated pharmacy service to the Frailty Team, supporting both the team in safe prescribing and medicines management, and its patients in medicines optimisation. You will also be a member of the Adult Medicine team in pharmacy and expected to contribute to the daily operations of the pharmacy department.
Main duties of the job
The successful candidate will be expected to:
· Work alongside specialist nurses, clinicians, and therapists in the frailty multidisciplinary team, supporting MDT colleagues in all pharmaceutical matters
· Provide expert advice on medicines to frailty colleagues
· Support frail and elderly patients to understand and manage their medications
· Undertake medication reviews and optimisation of medicine regimens for all patients seen by the frailty service during core working hours
· Liaise with primary care and community pharmacists, where appropriate, to communicate any changes in medications
· Manage supply of inpatient and discharge medications for frailty patients
· Undertake governance work, such as contribution to frailty-related guidelines and attendance at frailty-related meetings, as appropriate
· Contribute to core pharmacy services where needed, including dispensary work, late nights, on call, and ward cover
As the only pharmacist in the Frailty Team, this position will be yours to build. You will be supported by the wider Adult Medicine team and encouraged to undertake service improvement projects to build your portfolio. Educations, Training and Qualifications
Essential criteria
- Master’s degree in Pharmacy
- Registration with the GPhC
- Postgraduate qualification in hospital pharmacy (or in the process of obtaining one)
Desirable criteria
- Member of the RPS Faculty
- Member of the Royal Pharmaceutical Society
- Registered Independent prescriber
- Advanced Clinical Practitioner
Knowledge and Experience
Essential criteria
- 1-2 years post registration UK hospital experience
- Clinical experience in a range of ward/clinical areas
- Teaching/training/coaching
- Responsibility for a defined area requiring prioritisation/problem solving/delegation
- Experience of leading or implementing change
- Experience attending senior pharmacy meetings and an understanding of governance structure for that meeting
- Good clinical knowledge of commonly prescribed medicines
- Good knowledge of NICE guidelines for common conditions
- Good knowledge of the NHS and current priorities
- Experience managing datix investigations
Desirable criteria
- A good understanding of where pharmacy sits within the organisational structure
