Are you passionate about supporting patients in their own homes and committed to improving their quality of life?

We are seeking Generic Support Workers to join Selby Community Response Teams – to deliver high standards of multi-disciplinary interventions to patients within their own place of residence to prevent avoidable admissions to acute care and to support discharges. This role will be delivering interventions which support activities of daily living and promoting independence, as able.

Our well-established teams cover the historical city of York, market town of Selby as well as the rolling countryside within North Yorkshire. The team consists of Physiotherapists, Occupational Therapists, Nurses and Support Workers and works alongside Primary Care and Partners to provide Urgent Care Response, Frailty Hub as well as short term reablement/rehabilitation programmes.

Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required

Main duties of the job

You will undertake duties delegated by the Nursing, Physiotherapy and Occupational Therapy teams, and will be responsible for providing holistic, person centred, compassionate health care to patient’s 18 years and over, supporting rehabilitation, reablement and recovery within the comfort of the patients home.

We provide a 7 day service, between the hours of 8am - 8pm; therefore successful candidates will take part in a rota that covers the hours of the service, however we do offer a flexible and varied shift pattern. Qualifications

Essential criteria
  • A levels ( 3) or Level 3 national certificate or Level 3 diploma or Senior Healthcare support worker apprenticeship or Mathematics and English to Level 2: Functional Skills/GCSE Grade C
  • Proactively undertakes relevant specialist or departmental training in order to achieve required knowledge and competency for role.
Desirable criteria
  • Up to date personal portfolio of learning
  • Evidence of transferable workplace training and competencies

Skills

Essential criteria
  • Ability to problem solve, prioritise and escalate work related issues
  • Competent IT skills and the ability to use digital platforms and maintain business related information
  • Ability to use a range of communication, negotiation and motivation skills using a variety of formats including verbal, written and virtual.

Experience and Knowledge Required

Essential criteria
  • Experience of managing allocated tasks independently within own competency and taking responsibility for decision making
  • Experience of working within a team and with the general public
  • Practical awareness of issues affecting safety and risk

  • Experience of working within a health/social care or educational environment
