In North Yorkshire we have an exciting opportunity for you to join our 0-6 Team supporting with the delivery of the virtual element of our contract.
The post holder will be pro-active in delivering the national and Public Health agendas in response to identified health and social need, working in partnership with other agencies and meet agreed standards, objectives and statutory requirements.
Please note this vacancy role may require face to face patient management as well as virtual management and therefore the team members will be expected to conduct face to face assessments in line with service needs.
Main duties of the job
The successful candidate will work with parents, carers, children and partner agencies to promote the physical, emotional and social development of children.
Contributing to the delivery of the Healthy Child programme by undertaking some mandated contacts and targeted interventions as directed/delegated by the Specialist Community Public Health Nurse (SCPHN) and working in conjunction with the SCPHN to co-ordinate, plan, implement and evaluate a programme of care for children, young people and families.
They will support the completion of comprehensive health needs assessment of clients, families and community and facilitate the mobilisation of appropriate resources to meet identified needs.
The post holder will ensure inter-agency/ professional partnership working between services, statutory and voluntary organisations to facilitate integrated approaches in meeting the needs of children.
Excellent communication skills and a passion for public health are essential, alongside good organisational skills, flexibility to meet the needs of the service and ability to travel independently to meet service delivery as required.
This role will be predominantly virtual working.

HDFT are seeking to recruit Family Health Nurses to be part of the innovative 0-6 Service in North Yorkshire. We are the largest provider of children’s public health nursing services in the North East of England providing services that stretch from North Yorkshire up to Gateshead and Sunderland. We are dedicated to working towards improved public health outcomes for children and families in the footprint, delivering high quality universal and targeted health visiting and school health services to meet the key improvement priorities for the population. The 0-6 commissioned service within North Yorkshire has a blended approach to the provided contacts, with patients seen within their own homes, nursery settings and virtually using MS Bookings software. The role of the Family Health Nurse has a large proportion of virtual working, providing universal contacts across the whole of the North Yorkshire footprint, whilst also having an element of face to face working within the locality the family health nurse is based within. All practitioners are supported with a VPN which allows the virtual work to be completed from a base within the locality as well as at home working.

- Experience of working with children and families.

- Sound knowledge and experience of promoting the safeguarding and wellbeing of children.

- Experience of working in community setting
Qualifications
Essential criteria
- First level registration (RN Adult, RMN, RNLD, Midwife, RN Child)
Desirable criteria
- Post-registration qualification.
- ENB 998/730.
