This Service Manager post will be one of five working across the Harrogate & Rural Alliance (HARA) delivering integrated adult community service within people's home and also in the Harrogate District Hospital.  Our HARA community hubs bring together health and social care to keep people safe in their own homes.

Main duties of the job

To lead & manage a number of operational managers within the Harrogate and Rural Alliance and its aligned services. Ensuring that the care provided by community nursing, discharge nursing, therapy, social care assessment and provider staff is of high quality and wherever possible maximises opportunities for independence and recovery, are safe and take account of the strengths and assets of adults with care and support needs and their carers.

You will play a key role in developing services in response to National initiatives such as Ageing Well as well as responding to local needs. Working closely with health and social care clinical leads you will ensure that quality of care and patient and staff safety remain a priority and work within a financial envelope to deliver an efficient and effective service. You will demonstrate compassionate leadership, work in line with organisational values and behaviours and collaborate with wider stakeholders to develop innovative models of care.

Detailed job description and main responsibilities

This post provides an excellent opportunity to support the Alliance Director, other HARA Service Managers, Head of Nursing, Community Matron, Head of Practice and work collaboratively with other Hospital based Matrons, in managing and developing community services provided by Health and Social Care Services, including an additional portfolio of responsibilities detailed below, the post holder will be expected to:

  • Support to the Alliance Director, Community Matron, Head of Practice and other appropriate managers at NYCC and HDFT in the efficient and effective operational management of health and social care services
  • Be responsible for ensuring the services deliver improvements in Quality and Operational Targets, Health Care Standards and transformation plans and for developing a culture where quality and services continually improve in line with the HARA operating model.
  • Ensure an approach to the delivery and development of the services that improves service and outcomes for people.
  • Be responsible for the development and delivery of the work plan for HARA services.
  • Ensure by working with appropriate colleagues across the organisations and clinical commissioners that the services’ future shape and strategic direction are in accordance with the planned provision for local health and social care needs.
  • Working with colleagues to ensure that service and workforce plans are developed in accordance with current legislation and strategic plans. These are monitored and delivered against agreed performance criteria and will reflect the targets and priorities set out in national and local modernisation and performance targets. Take responsibility for budget management and the sustained delivery of key operational service targets.
  • Lead capacity and business planning across HARA and individual portfolio
  • Foster an effective style of leadership, partnership and integrated team working that will support improvements in staff and people’s experience
  • Maintain an approach to decision making, which ensures that the wider directorate, corporate and organisational agendas are appropriately reflected within the service strategic plans.
  • Line manage colleagues which could include but not restricted to Clinical Locality Managers, Acute Response Rehabilitation in the Community / Home / Hospital Service (ARCHS) Team Leads, Single Point of Access, End of Life Co-ordination, Community Discharge Hub, Social Care Community Teams, Team Managers, Reablement Registered Managers, Therapy Professional Lead and Administrators who manage discrete service areas.
  • Provide systems leadership and representation as part of inter-agency working, planning and developing services

Person specification

Experience

Essential criteria
  • • A professional social care or health qualification e.g. social work, occupational therapy, nursing, Registered Managers Award (NVQ4 Care.)
  • • Proven managerial and leadership experience within the NHS or public sector.
  • • A track record of leadership, performance improvement and organisational change.
Desirable criteria
  • • Management qualification or desire to work towards
