Job overview
We are looking for a motivated, patient focused pharmacist to join our specialist frailty team. We welcome applications from current Foundation Pharmacists who will gain automatic progression into the band 7 role once defined criteria is achieved. Applicants may be employed directly at Band 7 level if they demonstrate the necessary skills and experience.
The successful candidate will be a pharmacist with excellent communication and organisation qualities who has the strong interpersonal skills to allow them to work effectively as part of our highly regarded clinical team.
This novel post offers structured progression from Foundation to Specialist pharmacist in Frailty within a supportive clinical and multidisciplinary team. Whilst not exhaustive, this post holder will have the opportunity to develop pharmacy services to acute frailty and hospital at home patients.

Main duties of the job
To support the development, delivery and evaluation of the clinical pharmacy service to Frailty.
To participate, with support where appropriate, in Consultant led ward rounds.
To develop, deliver and evaluate training programmes for pharmacy staff and other members of the MDT managing Frailty patients.
To actively participate and develop a pharmacy related Quality Improvement programme within the Frailty specialty.
Embark on relevant training to support progression i.e. clinical diploma, independent prescribing.

Detailed job description and main responsibilities
To actively participate in a specialty Consultant led ward rounds and pharmacy led out-patient clinics.
To supervise and develop other members of the pharmacy team in the provision of pharmacy services to Frailty.
To provide expert pharmaceutical advice to members of the Medical Care Groups.
To work closely with the Lead Pharmacist to develop, deliver and evaluate the clinical pharmacy services to all frailty areas and wards.
Person specification
Qualifications
Essential criteria
- GPhC Registration
- MPharm Degree (4yrs) or equivalent
- Completed at least Year 1 of Post Graduate Diploma in Clinical Pharmacy/General Pharmacy Practice and studying for completion of year 2
Desirable criteria
- Independent prescriber
- Member of Royal Pharmaceutical Society
Experience
Essential criteria
- Experienced in managing patients in a range of specialties including elderly care and medicine
- Minimum of 1yr post registration hospital experience or equivalent clinical experience.
Desirable criteria
- Previous experience in developing guidelines/drug policies
