The successful candidate will gain a wide range of experience in clinical specialties including Frailty, Acute admissions, Surgery and Medicine and will be supported in developing to meet the RPS Foundation Framework standards.

Main duties of the job
- Providing pharmaceutical care to patients from admission to discharge on the wards
- Actively participating as part of the multi-disciplinary team at ward level
- Providing prescribing advice and support to medical staff
- Educating patients about their medicines
- Participation in quality improvement projects across the department and clinical areas
- Participating in late night and weekend working and on call rotas

Detailed job description and main responsibilities
- To clinically review prescriptions in both the ward and dispensary environments.
- To provide specialist pharmaceutical advice
- To accurately dispense and check prescriptions in the dispensary
- To conduct medication reviews at admission and discharge and implement changes to medicines as necessary and to accurately communicate these changes to primary care colleague

Qualifications
Essential criteria
- GPhC registered/GPhC provisional registration
- Master Degree in Pharmacy (4yrs) or equivalent
- Quality of supporting informaion
Desirable criteria
- MRPharmS
Experience

- Experience in working in multidisciplinary teams

- Hospital pharmacy experience
- Community pharmacy experience

You must have appropriate UK professional registration.
The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
